TRT for Men with Diabetes in Dubai
TRT for men with diabetes is simple with Dr Felix's personal care. There is no testosterone sold as a blood sugar cure, no diagnosis from a test that ignores your weight, and your diabetes and hormones are looked at together.
Your first visit includes testosterone, HbA1c and your current diabetes medicines. Dr Felix checks whether your testosterone is truly low or only looks low because of diabetes and weight.

The right test with diabetes
Diabetes and excess weight lower the carrier protein SHBG. Free testosterone shows whether the level is truly low.
Honest about blood sugar
Testosterone treats deficiency symptoms. Blood sugar is treated with diabetes care and weight loss.
Hormones and diabetes, one doctor
This is a personal practice. Dr Felix Happich, German-trained and licensed by the Dubai Health Authority, looks after both himself.

What does TRT for men with diabetes include?
TRT (testosterone replacement therapy) for men with diabetes with Dr Felix includes 6 parts. The plan confirms a true deficiency first and keeps your blood sugar, heart and blood count in view.
- Testosterone: fasting between 8 and 10 in the morning on two days, because food and sugar lower it.
- Free testosterone: measured by equilibrium dialysis when weight or diabetes lower SHBG.
- LH and FSH: the brain signals to the testes, often low in type 2 diabetes.
- Blood sugar: HbA1c (glycated haemoglobin, the average blood sugar over three months) and your current medicines.
- Blood count and prostate: haematocrit, and PSA from age 50.
- Sleep: snoring and daytime sleepiness, because severe sleep apnoea worsens on testosterone.

How common is low testosterone in men with diabetes?
Low testosterone is common in men with type 2 diabetes. In a study of 103 men with type 2 diabetes, one in three had low free testosterone, mostly with low brain signals (Journal of Clinical Endocrinology and Metabolism, 2004).
For this reason, the Endocrine Society lists type 2 diabetes among the conditions in which testosterone is measured. Weight is part of the link: the higher the body mass index, the lower the testosterone.
Does TRT lower blood sugar?
No, TRT does not lower blood sugar in men with diabetes and low testosterone. In a TRAVERSE substudy of 3,880 men with diabetes and 1,175 with prediabetes, testosterone gel did not improve HbA1c or glucose and did not prevent diabetes compared with placebo (JAMA Internal Medicine, 2024).
A different trial, T4DM, gave 1,007 men aged 50 to 74 in a lifestyle programme testosterone injections or placebo. After 2 years, type 2 diabetes was present in 12 percent with testosterone and 21 percent with placebo, but the haematocrit rose and these men had no confirmed deficiency (The Lancet Diabetes and Endocrinology, 2021). Testosterone treats deficiency symptoms. Diabetes stays treated with diabetes care.
What happens after the first visit?
After the first visit, testosterone and blood sugar are reviewed on one schedule. Diabetes medicines stay in place, and testosterone is added only for a confirmed deficiency.
- Within 2 weeksSecond morning testosterone, free testosterone, LH and FSH
- Results visitDiagnosis, and a decision on TRT or on weight and sleep first
- After 2 to 3 monthsTestosterone level, symptoms and HbA1c reviewed
- Every 6 to 12 monthsTestosterone, haematocrit, HbA1c and PSA from age 50
What else raises testosterone with diabetes?
Weight loss raises testosterone in men with diabetes and excess weight. A review of 24 studies found that weight loss raises both total and free testosterone in men with obesity (European Journal of Endocrinology, 2013).
- Weight: losing excess weight raises testosterone and lowers blood sugar.
- Sleep apnoea: treated when present, because it adds to tiredness.
- Blood sugar: better control helps energy and erections.
- Medicines: long-term opioids and steroid tablets are reviewed.

More questions about TRT for men with diabetes
What does TRT for men with diabetes cost?
The cost of TRT for men with diabetes depends on the blood tests you need and whether treatment becomes part of your plan. You get the full cost in the consultation before anything starts.
Is my testosterone low because of my weight?
Often, yes. Excess weight lowers SHBG, the protein that carries testosterone, so total testosterone looks low while free testosterone stays normal. Free testosterone by equilibrium dialysis shows the difference.
Is TRT safe with diabetes and heart risk?
In the TRAVERSE trial, men with heart disease or heart risk, most of them with obesity and many with diabetes, had no rise in heart attacks or strokes on testosterone. Atrial fibrillation and pulmonary embolism were more common.
Does TRT help erections with diabetes?
Partly. Testosterone improves desire more than erections, and erectile problems in diabetes often come from the blood vessels, which is why erections are assessed separately.
