What does a women’s sexual health consultation with Dr Felix include?
A women’s sexual health consultation with Dr Felix includes 4 areas: desire, arousal, orgasm and pain. Low sexual desire is common, but it becomes a disorder only when it causes personal distress.
- Desire: low interest in sex, see low libido in women.
- Arousal and orgasm: reduced arousal, lubrication or orgasm.
- Pain: pain with sex from vaginal dryness or a tight pelvic floor.
- Causes: medicines, mood, hormones, other illness, tiredness and the relationship.
In the PRESIDE study of 31,581 women in the United States, 39 percent reported low desire, but only about 10 percent had low desire with distress, about 1 in 10 (Obstetrics and Gynecology, 2008).

How is painful sex treated?
Painful sex is treated by its cause, and the two common causes have different treatments. Pain from vaginal dryness after menopause is treated with lubricants, moisturisers or low-dose vaginal oestrogen; pain from a tight pelvic floor is treated with pelvic floor physiotherapy and vaginal dilators.
- Vaginal dryness: non-hormonal moisturisers first, then vaginal oestrogen, which stays local and continues long term.
- Tight pelvic floor: pelvic floor physiotherapy with dilator training.
- Infection or skin disease: treated before anything else.
Desire and arousal are reassessed after the pain has settled, not before.
Does testosterone help women with low desire?
Testosterone helps one group of women: postmenopausal women with low desire that causes distress, after other causes are treated. The Global Consensus Position Statement on testosterone for women, endorsed by 11 medical societies, supports testosterone only for postmenopausal women with hypoactive sexual desire disorder (HSDD), low desire with distress (Journal of Clinical Endocrinology and Metabolism, 2019).
- Form: transdermal testosterone at a dose that keeps blood levels in the normal female range.
- Monitoring: a blood level before starting and after 3 to 6 weeks.
- Stop rule: treatment stops after 6 months when desire has not improved.
- Not used: in premenopausal women, and not for tiredness or mood alone.
Does the type of HRT affect libido?
Yes, the route of oestrogen in HRT affects libido. Oestrogen tablets raise sex hormone-binding globulin (SHBG), the protein that binds testosterone, and lower free testosterone; oestrogen patches and gels do not.
Women on HRT tablets with low desire are therefore often switched to a patch or gel first. HRT options: HRT patch or oestrogen gel. Desire after menopause: low libido after menopause.
Women’s sexual health in Dubai and the UAE
Many women in Dubai raise sexual concerns late, often only when asked directly. The consultation with Dr Felix is confidential, and cultural or religious concerns about sexuality are part of the conversation, not a barrier to it.
Pelvic floor physiotherapy and sex therapy are available in Dubai, and Dr Felix refers to them when they fit the cause.
