In short

Vaginal oestrogen is a low-dose estradiol or estriol tablet, cream or ring that treats vaginal dryness, pain during sex and recurrent urinary infections after menopause. Symptoms usually improve within 2 to 4 weeks. Blood oestrogen levels stay in the normal postmenopausal range, so no progesterone is needed and long-term use is possible.

Treatment Overview

What is vaginal oestrogen?

Vaginal oestrogen is a low dose of oestrogen placed in the vagina, where it restores the thickness, moisture and acidic pH of the tissue. Low-dose vaginal oestrogen is the most effective treatment for moderate to severe symptoms that do not settle with moisturisers and lubricants, according to clinical guidance.

Vaginal oestrogen is local therapy. It does not treat hot flushes; those need systemic HRT through the skin or HRT tablets.

Which symptoms does vaginal oestrogen treat?

Vaginal oestrogen treats the genitourinary syndrome of menopause, which affects about 50 percent of women after menopause, 1 in 2:

  • Vaginal: dryness, burning, itching and discharge from thin tissue
  • Sexual: lack of lubrication and pain during sex
  • Urinary: urgency, burning and recurrent urinary tract infections

Vaginal oestrogen also reduces recurrent urinary infections and overactive bladder symptoms. Leakage of urine usually needs other treatment. Low sexual desire has its own causes: low libido in women.

Which forms of vaginal oestrogen exist?

Tablets, creams, pessaries and rings work equally well in a systematic review of 19 trials with over 4,000 women. The table compares the forms.

Forms of vaginal oestrogen
Form How it is used
Vaginal tablet (estradiol 10 micrograms) Daily for 2 weeks, then twice a week
Cream or gel (estradiol or estriol) Daily for 2 weeks, then twice a week; also treats the vulva
Pessary or ovule (estriol) Daily at first, then twice a week
Ring (estradiol 7.5 micrograms per day) Changed every 90 days, about every 3 months

Cream suits women with sore or cracked vulval skin; a tablet or ring suits long-term use. Availability of each product varies between Dubai pharmacies, and Dr Felix prescribes a form in local stock.

How is vaginal oestrogen used?

Vaginal oestrogen is used daily for the first 2 weeks, then twice a week as maintenance. Improvement usually starts within 2 to 4 weeks. Most women find the lowest dose that keeps symptoms away and stay on it.

Lubricants during sex and a vaginal moisturiser two or three times a week are added as needed. Pelvic floor exercises support the treatment.

Is vaginal oestrogen safe long term?

Low-dose vaginal oestrogen raises blood oestrogen only minimally, within the normal postmenopausal range. Observational data over several years show no increase in breast or endometrial cancer, stroke or blood clots. In November 2025 the US Food and Drug Administration asked manufacturers to remove the boxed warning from oestrogen products, including low-dose vaginal oestrogen.

No progesterone is needed with low-dose vaginal oestrogen. Any bleeding after menopause is assessed, also during treatment. After breast cancer, vaginal oestrogen is used only after non-hormonal options fail and together with the oncologist.

What are the alternatives to vaginal oestrogen?

Vaginal moisturisers and lubricants are the first step for mild symptoms. Other prescription options are:

  • Vaginal DHEA (prasterone): a daily pessary for pain during sex, for women who prefer to avoid oestrogen
  • Ospemifene: a daily tablet for women who cannot use a vaginal product, with a small risk of hot flushes

Laser and radiofrequency devices are not recommended, because sham-controlled trials show no clear benefit over placebo.

Vaginal oestrogen in Dubai and the UAE

Why do many women in the UAE not ask about vaginal dryness?

Many women with genitourinary symptoms never raise them with a doctor because of embarrassment, cultural or religious beliefs, or not knowing a safe treatment exists, according to clinical guidance. In a Dubai study of 591 women interviewed in 2020, 82 percent had fair or poor knowledge of menopause, about 8 in 10, and 77 percent did not know when hormone therapy is unsuitable (Menopause, 2021). A 2026 review of 15 studies from Arab countries found hormone therapy use below 20 percent, fewer than 1 in 5 women, in 11 of them, with fear of risks and a preference for herbal remedies as main barriers (Frontiers in Global Women’s Health).

How private is a consultation about vaginal symptoms?

The consultation is private, one to one, with a chaperone on request. An examination happens only with consent and only when it changes the treatment, for example with bleeding or unusual discharge.

Dr Felix treats within the scope of his Dubai Health Authority licence. When a case needs a specialist view, for example after breast cancer or with a clotting disorder, the treatment decision is made together with an endocrinologist, gynaecologist or oncologist.

Benefits, Limits & Safety

What are the benefits of vaginal oestrogen?

Vaginal oestrogen restores vaginal pH, moisture and tissue thickness, relieves pain during sex and reduces recurrent urinary infections. It is the most effective treatment for moderate to severe symptoms in clinical guidance.

What are the limits of vaginal oestrogen?

Vaginal oestrogen does not treat hot flushes, bone loss or low desire, and symptoms return after stopping. Side effects are uncommon: local irritation, spotting or breast tenderness. Clinical trials on the womb lining run up to 1 year; longer safety data come from observational studies.

What the evidence shows

Tablets, creams and rings work equally well in 19 trials with over 4,000 women; improvement within 2 to 4 weeks.

How you are followed

Review after about 3 months, then yearly: symptoms, dose and any bleeding.

When vaginal oestrogen is not used first

Unexplained bleeding after menopause, and breast cancer on treatment, where non-hormonal options and the oncologist come first.

Cost & Program Investment

What treatment with vaginal oestrogen costs depends on the form and how often it is used. Dr Felix explains the figure before any prescription.
