In short

HRT patches and oestrogen gel deliver estradiol through the skin and are the first choice for most women starting menopausal hormone therapy, because they carry a lower risk of blood clots and stroke than tablets. Standard doses stop hot flushes completely in about 80 percent of women, 8 in 10, usually within 3 to 4 weeks. Women with a uterus add progesterone.

Treatment Overview

What are HRT patches and oestrogen gel?

HRT patches and oestrogen gel are transdermal estradiol, the same hormone the ovaries made before menopause, absorbed through the skin into the blood. Estradiol is body-identical, so the patch and the gel are regulated bioidentical hormone therapy. The overview of every HRT form, with benefits and risks: hormone replacement therapy.

Why is estradiol through the skin preferred?

Estradiol through the skin is preferred because it carries a lower risk of venous thrombosis and stroke than oral estradiol, according to clinical guidance. Estradiol absorbed through the skin bypasses the first pass through the liver, the step that raises clotting factors with tablets.

The skin route is the clear choice with high triglycerides, gallbladder disease, migraine, a known low-risk clotting tendency such as heterozygous factor V Leiden, a moderate 10-year cardiovascular risk of 5 to 10 percent, or epilepsy medicines that speed up liver breakdown of hormones.

Should you choose a patch or a gel?

A patch and a gel work equally well and differ in routine. The table compares the two forms.

HRT patch and oestrogen gel compared
Patch Gel
How often Changed once or twice a week, depending on the brand Rubbed into the skin once a day
Where Lower abdomen or buttock, away from the breasts Arms, shoulders or inner thighs, as the product leaflet states
Dose adjustment Fixed strengths, changed by switching patch size Number of pumps or sachets changed
Typical problem Skin irritation or loosening with heat, sweat or swimming Drying time before dressing, daily routine

Patches loosen more easily in Dubai heat, at the pool and during sport. A patch that falls off is replaced with a new one, as the product leaflets advise. A gel suits women whose skin reacts to adhesive.

What dose of estradiol is used through the skin?

Treatment starts low, usually with a 0.025 mg per day patch for moderate symptoms or 0.05 mg per day for severe symptoms, and the dose rises until symptoms settle, according to clinical guidance. The 0.05 mg patch is the standard dose, equal in effect to 1 mg of oral estradiol. Gel doses follow the same targets in pumps or sachets.

Women under 45 after removal of both ovaries often need a higher dose, such as 0.1 mg per day, until the average age of menopause at 51: early menopause treatment.

Why do most women need progesterone with the patch or gel?

Every woman with a uterus takes a progestogen with systemic estradiol to protect the womb lining, because estradiol alone thickens the lining within about 6 months and raises the risk of endometrial cancer. After a hysterectomy, estradiol alone is used: HRT after hysterectomy.

Progesterone regimens with transdermal estradiol
Stage Usual progesterone plan
Perimenopause or first 2 to 3 years after the last period Micronised progesterone 200 mg at bedtime on 12 days of each month, with a monthly bleed
More than 2 to 3 years after the last period Micronised progesterone 100 mg at bedtime every day, usually without bleeds
Progesterone not tolerated Levonorgestrel coil or vaginal progesterone, decided individually

Micronised progesterone is the first choice in clinical guidance and is taken at bedtime because it makes some women sleepy. Some combined patches contain estradiol and a progestin together.

What happens in the first weeks on a patch or gel?

Hot flushes usually ease within 3 to 4 weeks, about one month. Breast tenderness is common at the start and falls with lower doses. Irregular bleeding is common in the first 6 months of a continuous regimen and is followed up; bleeding beyond 6 months needs an assessment of the womb lining: bleeding on HRT.

HRT patches and gel in Dubai and the UAE

Where do HRT patches and gel come from in Dubai?

Estradiol patches and gels are prescription medicines dispensed by licensed pharmacies in Dubai. Brand names and strengths in stock vary between pharmacies, so Dr Felix prescribes a strength that is available locally and names an equivalent if supply changes.

How well known is HRT among women in the UAE?

In a Dubai study of 591 women interviewed in 2020, 82 percent had fair or poor knowledge of menopause, about 8 in 10, and 77 percent did not know when hormone therapy is unsuitable (Menopause, 2021). A 2026 review of 15 studies from Arab countries found hormone therapy use below 20 percent, fewer than 1 in 5 women, in 11 of them, with fear of risks and a preference for herbal remedies as main barriers (Frontiers in Global Women’s Health).

Can you travel with HRT patches and gel?

Hormone medicines brought into the UAE from abroad travel with the prescription and a doctor’s letter, and controlled medicines need a permit before entry, according to the UAE Embassy guidance. Dr Felix issues a Dubai prescription so treatment continues with local pharmacy supply.

Dr Felix treats within the scope of his Dubai Health Authority licence. When a case needs a specialist view, for example after breast cancer or with a clotting disorder, the treatment decision is made together with an endocrinologist, gynaecologist or oncologist.

Who Is a Good Candidate?

  • Women with moderate to severe hot flushes, night sweats or menopause-related sleep and mood problems, under 60 or within 10 years of their last period.
  • Women with migraine, high triglycerides, gallbladder disease or a moderate cardiovascular risk, for whom tablets are less suitable.
  • Women under 45 with early or surgical menopause, who need oestrogen until the average age of menopause.
  • Women who prefer a weekly patch or a daily gel to a daily tablet.
  • Previous breast cancer, a blood clot, a stroke or a transient ischaemic attack: systemic HRT is not used.
  • Coronary heart disease or active liver disease: systemic HRT is not used.
  • Unexplained vaginal bleeding: assessed before any hormone is started.
  • A 10-year cardiovascular risk above 10 percent, or a raised 5-year breast cancer risk: non-hormonal treatment first.

Program Structure & Follow-Up

How is treatment with a patch or gel followed up?

The first review is about 3 months after starting, then once a year, in line with NICE guidance. Each review covers symptom relief, bleeding pattern, blood pressure, breast screening and whether the dose still fits. Dose changes follow symptoms, not blood levels, for most women.

Hot flushes return in many women when estradiol stops suddenly, so a stop is planned as a gradual taper. Continuous care between visits: Women’s Hormone Membership.

Benefits, Limits & Safety

What are the benefits of HRT patches and gel?

Estradiol through the skin reduces hot flushes by about 75 percent in frequency, 3 in 4 fewer, in pooled trials and stops them fully in about 80 percent of women at standard doses. It also improves sleep disturbed by night sweats, mood swings in perimenopause and bone density while it is used.

What are the risks of HRT patches and gel?

Transdermal estradiol at standard doses does not raise the risk of blood clots in observational data, unlike oral estradiol. Combined therapy with a progestogen raises breast cancer risk slightly after several years; micronised progesterone is chosen partly for this reason. The detailed risk comparison: HRT benefits and risks. Women with a strong family history of breast cancer: HRT with a family history of breast cancer.

What the evidence shows

About 75 percent fewer hot flushes in pooled trials; complete relief in about 80 percent of women at standard doses.

How you are followed

Review after about 3 months, then yearly: symptoms, bleeding, blood pressure, breast screening and dose.

When patches and gel are not used

Previous breast cancer, a blood clot or stroke, coronary heart disease, active liver disease or unexplained vaginal bleeding.

Cost & Program Investment

What treatment with an HRT patch or gel costs depends on the product, the dose and the progesterone plan. Dr Felix explains the figure before any prescription.

Frequently Asked Questions

An HRT patch carries a lower risk of blood clots and stroke than HRT tablets, according to clinical guidance, because estradiol through the skin bypasses the liver. Both relieve hot flushes equally well.

Hot flushes usually ease within 3 to 4 weeks on an HRT patch or gel. The dose is raised if symptoms persist after that time.

Yes, every woman with a uterus takes a progestogen with an oestrogen patch or gel to protect the womb lining. After a hysterectomy, oestrogen alone is used.

A patch that falls off is replaced with a new one, as the product leaflets advise. Women whose patches loosen often in Dubai heat or at the pool switch to oestrogen gel.

The cost of HRT patches or gel in Dubai depends on the brand, the dose and the progesterone added. Dr Felix explains the full cost in the consultation before anything is prescribed.
