What does an HRT review with a family history of breast cancer include?
An HRT review with a family history of breast cancer with Dr Felix includes 6 parts. The review turns your family history into a personal risk figure before any decision.
- Family history: who was affected, on which side, and at what age.
- Risk estimate: your 5-year breast cancer risk from a validated tool.
- Screening: your latest mammogram and breast findings.
- Personal factors: weight, alcohol and earlier breast biopsies.
- Symptoms: how much hot flushes and poor sleep affect your life.
- Alternatives: non-hormonal treatments when HRT does not fit.

Can I take HRT with a family history of breast cancer?
Yes, many women with a family history of breast cancer take HRT. HRT does not add extra risk on top of a family history, but the absolute risk is higher because the starting risk is higher.
This extra risk is similar in size to the risk from obesity or low physical activity. Women with a BRCA variant whose ovaries were removed had no further rise in breast cancer risk with HRT. A personal history of breast cancer rules out HRT through the whole body.
When is HRT not advised with a family history?
HRT is not advised when your own breast cancer risk is moderate or high. The Endocrine Society suggests non-hormonal treatment for women with a 5-year breast cancer risk of 1.67 percent or more.
Risk tools are less accurate for women with several affected first-degree relatives. In these families, genetic counselling comes first. Non-hormonal medicines treat hot flushes well for many women.
Which HRT carries less breast cancer risk?
The type of HRT changes the breast cancer risk. In a 2019 analysis of 108,647 women with breast cancer, the risk rose with years of use and was higher with combined HRT than with oestrogen alone (The Lancet, 2019).
- Oestrogen alone: for women after hysterectomy, lower breast risk than combined HRT.
- Micronised progesterone: limited data suggest less risk than older progestins.
- Vaginal oestrogen: not linked to extra breast cancer risk.
- Duration: the need for HRT is reviewed every year.
What else lowers breast cancer risk?
Everyday factors change breast cancer risk as much as combined HRT does. Weight, activity and alcohol are the three factors under your control.
- Weight: keep a healthy weight after menopause.
- Activity: exercise regularly.
- Alcohol: drink less or not at all.
- Screening: keep mammograms at the interval set for your risk.

More questions about HRT and breast cancer in the family
What does an HRT review with a family history of breast cancer cost?
The cost of an HRT review with a family history of breast cancer depends on the tests you need and which treatment becomes part of your plan. You get the full cost in the consultation before anything starts.
My mother had breast cancer. Can I take HRT?
Often, yes. A mother with breast cancer raises your starting risk, and HRT adds a similar relative amount as for other women. Your personal numbers decide.
Does the extra risk go away after stopping HRT?
The extra risk falls after stopping, but part of it remains for years after long use of combined HRT.
Are bioidentical hormones safer?
Body-identical patches and micronised progesterone likely carry less risk than older products. Custom-mixed compounded hormones are not tested in trials and are not recommended.
