What does HRT for women over 60 include?

HRT (hormone replacement therapy) for women over 60 with Dr Felix includes 6 checks. The balance of benefit and risk is less favourable after 60 than in the 50s, so each risk is measured.

  • Symptoms: hot flushes, sleep, mood and vaginal dryness.
  • Heart: blood pressure, cholesterol and your 10-year cardiovascular risk.
  • Breast: your latest mammogram and your family history.
  • Clot risk: earlier clots, weight and mobility.
  • Bones: fracture risk and bone density.
  • Current HRT: type, dose and route if you already take it.

Can I start HRT after 60?

Starting HRT after 60 is possible, but only after an individual risk review. In the Women’s Health Initiative, risks were higher in women who started at 60 or later, or more than 10 years after menopause.

27.1% vs 27.6%deaths over 18 years with hormone therapy against placebo in 27,347 women.Women’s Health Initiative, JAMA 2017

Overall mortality stayed the same with hormone therapy and placebo (JAMA, 2017). The American Geriatrics Society advises against starting at 65 or older. The trial tested older tablets, and patches with micronised progesterone likely carry lower risks.

Can I keep taking HRT after 60?

Yes, many women keep taking HRT after 60 when hot flushes return without it. More than 40 percent of women aged 60 to 65 still have hot flushes, and extended use is reasonable when the benefits outweigh the risks.

Continuing means the lowest dose that controls symptoms, often a patch or gel instead of tablets, and a planned attempt to reduce later. Stopping abruptly brought back moderate to severe symptoms in more than half of women with hot flushes in the Women’s Health Initiative.

Why does the form of oestrogen matter after 60?

The form of oestrogen changes the risk of clots and stroke after 60. Oestrogen tablets raise the risk of clots and stroke, while low-dose patches do not appear to raise either.

  • Tablets: pass through the liver and raise clot risk.
  • Patches up to 50 micrograms: no higher stroke risk in a study of about 16,000 strokes.
  • Higher patch doses: stroke risk rose above 50 micrograms.
  • Micronised progesterone: likely lower clot risk than older progestins.

What else helps women over 60?

Not every symptom after 60 needs HRT through the whole body. Low-dose vaginal oestrogen treats dryness and urinary symptoms and was the only type of HRT not linked to extra breast cancer risk (The Lancet, 2019).

  • Sleep: other causes such as sleep apnoea checked.
  • Bones: exercise, vitamin D and bone medication when needed.
  • Heart: blood pressure and cholesterol treated on their own.
  • Yearly review: dose, form and whether to continue.

What does HRT for women over 60 cost?

The cost of HRT for women over 60 depends on the tests you need and which treatment becomes part of your plan. You get the full cost in the consultation before anything starts.

Do I have to stop HRT at 60?

No. There is no fixed age to stop HRT. The decision is reviewed every year based on your symptoms and your personal risks.

Will my hot flushes come back if I stop?

Often, yes. Hot flushes return in many women after stopping, and tapering over several months is one option to soften the change.

Is vaginal oestrogen safe after 60?

Yes, low-dose vaginal oestrogen acts mainly locally. In a 2019 analysis of 108,647 women with breast cancer, it was the only HRT type not linked to extra breast cancer risk.
