What does insulin resistance treatment include?

Insulin resistance treatment with Dr Felix starts with your blood values. Your plan is built on your HbA1c, your body composition and what you can keep up in daily life.

  • Blood work: HbA1c, fasting glucose, cholesterol, liver and kidney values.
  • Body composition and waist: not only the number on the scale.
  • Food: no special diet required, an eating pattern that fits your routine.
  • Movement: at least 150 minutes a week, ideally cardio plus weights.
  • Medication: only with a medical reason.
  • Retesting: HbA1c and weight at fixed review dates.

Does losing weight reverse prediabetes?

Weight loss is the strongest lever against prediabetes. In the Diabetes Prevention Program, adults with prediabetes who lost about 7 percent of their weight lowered their risk of type 2 diabetes by 58 percent over three years (New England Journal of Medicine, 2002). Each kilo lost lowered the risk by about 16 percent.

Getting blood sugar back into the normal range counts, even if only for a while. In the same study, people whose glucose tolerance returned to normal at least once developed diabetes half as often over six years, 16 compared with 32 percent.

Can medication help with insulin resistance?

For adults with prediabetes and obesity, weight loss injections lower the risk of type 2 diabetes. In a trial of about 1,000 adults with obesity and prediabetes, 1.3 percent on tirzepatide (Mounjaro) developed type 2 diabetes over about three and a half years, compared with 13.3 percent on placebo. Some of the benefit faded within 17 weeks of stopping.

Metformin is the older and cheaper tablet with long-term data. In the Diabetes Prevention Program it lowered the risk by 31 percent. Weight loss injections are considered at a BMI of 30 or more, or 27 or more with a weight-related condition, alongside nutrition and activity support, after an individual assessment of prior efforts, clinical needs and the applicable product information.

How is progress measured?

Progress with insulin resistance is measured in blood values and body composition, not in how strict the diet feels. The first target is a 5 percent weight loss, and the long-term goal is an HbA1c back in the normal range. People with prediabetes who take metformin need their HbA1c checked at least once a year.

  2. After 3 to 6 monthsReview: is the 5 percent target reached, is medication needed
  3. At least once a yearHbA1c and glucose repeated

What else helps with insulin resistance?

Weight loss does most of the work, and a few other habits add to it. In a study of over 32,000 men, 150 minutes a week of weight training or aerobic exercise was linked to a lower risk of type 2 diabetes, with the biggest drop when both were combined.

  • Diet pattern: Mediterranean, plant-based and DASH diets are linked to lower diabetes risk. Whether they work without weight loss is unclear.
  • Carbohydrates: low-carb is fine if it suits you, but no diet has to be followed exactly.
  • Vitamin D: a reasonable, cheap addition when your level is low. The effect is small.
  • Surgery: an option for severe obesity. In a Swedish study, diabetes developed 4 times less often over 15 years after surgery than with usual care.

More questions about insulin resistance and weight loss

What does insulin resistance treatment cost?

The cost of insulin resistance treatment depends on the blood tests you need and whether medication becomes part of your plan. You get the full cost in the consultation before anything starts.

My blood sugar is normal. Is this still for me?

Yes, if your weight keeps rising or your waist is growing. HbA1c, glucose, cholesterol and liver values show early changes, and a normal result is also useful as a starting point.

Do I have to take medication forever?

Not always. Weight loss injections work only while you take them, and weight usually returns after stopping. Metformin is safe long term, and some people can stop it after reaching their targets.

How likely is prediabetes to become diabetes?

About 5 to 10 percent of adults with prediabetes develop type 2 diabetes each year. Weight loss, exercise and, where needed, medication lower that risk.
