In short

Metabolic weight loss is a weight programme built around blood sugar, insulin resistance, blood lipids, blood pressure and liver health. It suits people with abdominal fat and borderline results. Weight loss is the main treatment for metabolic syndrome and lowers the risk of type 2 diabetes and heart disease.

Treatment Overview

What is metabolic syndrome?

Metabolic syndrome is the combination of abdominal fat, raised blood sugar, abnormal blood lipids and high blood pressure. Clinical guidance links it to insulin resistance, meaning the body responds less to insulin. Together, these factors raise the risk of type 2 diabetes and cardiovascular disease. The same pattern can occur in people with abdominal fat whose total weight looks normal.

How is metabolic syndrome diagnosed?

Metabolic syndrome is usually diagnosed when at least three of five criteria are met, according to the most widely used definition (NCEP ATP III).

Criteria for metabolic syndrome (NCEP ATP III)
Criterion Threshold
Waist circumference Over 102 cm in men, over 88 cm in women
Triglycerides 150 mg/dL (1.7 mmol/L) or more, or on treatment
HDL cholesterol Below 40 mg/dL (1.0 mmol/L) in men, below 50 mg/dL (1.3 mmol/L) in women, or on treatment
Blood pressure 130/85 mmHg or more, or on treatment
Fasting glucose 100 mg/dL (5.6 mmol/L) or more, or on treatment

Other definitions use lower waist thresholds for people of Asian background.

Which tests are done?

The assessment measures waist, blood pressure and body composition, plus fasting glucose, HbA1c, lipids, liver, kidney and thyroid function. A single fasting glucose can look normal while other values already show risk. Looking at the full pattern together gives a clearer picture than one number.

How is metabolic syndrome treated?

The core treatment is weight loss and more physical activity, according to clinical guidance. These measures lower the risk of type 2 diabetes, cardiovascular events and death.

  • Diet: clinical guidance suggests a Mediterranean or low-fat diet for metabolic syndrome.
  • Exercise: combined aerobic and strength training appears most beneficial for metabolic values.
  • Protein: 1 to 2 g per kg of ideal body weight daily helps protect muscle during weight loss.
  • Blood pressure and lipids: treated to targets that fit the individual cardiovascular risk, sometimes with medication earlier than in lower-risk people.
  • Weight medication: considered when lifestyle changes are not enough, for example a GLP-1-based medicine.

Can prediabetes be reversed?

The risk of progressing from prediabetes to type 2 diabetes can be cut substantially with weight loss. In the Diabetes Prevention Program, a lifestyle programme aiming for 7 percent weight loss and 150 minutes of weekly exercise reduced new diabetes by 58 percent over about three years, compared with 31 percent for metformin.

Metabolic health in Dubai and the UAE

About 22 percent of adults in the UAE live with obesity, according to the national health survey. The survey figures are on the medical weight loss page.

For adults of South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean background, NICE sets obesity at a BMI of 27.5 rather than 30, which matters when metabolic values are borderline. The thresholds in full.

How is Ramadan handled with diabetes medication?

People taking insulin or sulfonylureas need an individual plan before Ramadan, because fasting raises the risk of low blood sugar, according to clinical guidance on religious fasting. The plan is agreed at a review before the fast.

Metabolic weight loss starts from a measurable condition: insulin resistance, prediabetes weight gain or metabolic syndrome. How Dr Felix treats weight together with these conditions: insulin resistance treatment, weight loss injections for type 2 diabetes and weight loss injections for fatty liver.

Who Is a Good Candidate?

  • Adults with abdominal fat and borderline blood sugar, lipids or blood pressure.
  • Adults with prediabetes who want to lower their risk of type 2 diabetes.
  • Adults with fatty liver found on a scan or blood test.
  • Adults of South Asian, Middle Eastern or other backgrounds with higher risk at a lower BMI.
  • Very high blood sugar or symptoms such as heavy thirst and weight loss without trying: needs prompt diabetes assessment first.
  • Pregnancy: metabolic targets are set differently.
  • Signs of an eating disorder: specialist care comes first.

Program Structure & Follow-Up

How is progress measured?

Progress is measured by the values that were abnormal, not only by weight. The first review is after about 4 to 6 weeks, then about every 2 to 3 months. HbA1c reflects average blood sugar over about three months, so it is usually repeated at around three months. New questions between appointments go through the membership programmes.

How much weight loss is needed?

Clinical guidance reports health benefits from about 5 percent weight loss. In the Look AHEAD trial of adults with type 2 diabetes, those who lost more than 10 percent in the first year had a 20 percent lower risk of cardiovascular events.

Why is sleep part of the assessment?

Obstructive sleep apnoea is common with obesity and improves with weight loss. If your history points to it, testing for it is discussed.

Benefits, Limits & Safety

What are the benefits?

Weight loss and exercise lower the risk of type 2 diabetes and improve blood pressure, lipids and liver fat, according to clinical guidance. Intensive programmes such as the DiRECT trial reported remission of type 2 diabetes in about half of participants.

What safety points matter?

  • Diabetes medicines: insulin and sulfonylureas often need lower doses as weight falls, otherwise blood sugar can drop too low.
  • Blood pressure medicines: may need adjusting as blood pressure improves.
  • Gallstones: the risk rises with rapid weight loss.
  • Very low carbohydrate diets: not suitable with some medicines and decided individually.

What are the limits?

Clinical guidance notes that experts disagree on whether metabolic syndrome is a separate disease or a cluster of risk factors. Either way, each factor is treated. Not every borderline value needs medication; some are monitored.

What the evidence shows

Lifestyle-based weight loss cut new type 2 diabetes by 58 percent in the Diabetes Prevention Program. Benefits start at about 5 percent weight loss.

How you are followed

Reviews after 4 to 6 weeks, then about every 2 to 3 months, with repeat HbA1c, lipids and liver values at agreed points.

When this route is not right

Very high blood sugar with symptoms needs prompt diabetes care first. Pregnancy and eating disorders need a different approach.

Cost & Program Investment

What the metabolic work-up costs depends on which blood tests your history calls for. The figure is agreed with you in the consultation.

Frequently Asked Questions

Metabolic syndrome is the combination of abdominal fat, raised blood sugar, abnormal blood lipids and high blood pressure. It raises the risk of type 2 diabetes and heart disease.

It is usually diagnosed when three of five criteria are met: large waist, high triglycerides, low HDL cholesterol, high blood pressure and raised fasting glucose.

Fasting glucose, HbA1c, lipids, liver, kidney and thyroid function, plus waist, blood pressure and body composition.

Yes. Clinical guidance notes that the same risk pattern can occur with abdominal fat even when total body weight is normal.

Clinical guidance suggests a Mediterranean or low-fat diet for metabolic syndrome, combined with aerobic and strength training.

Weight loss cut new diabetes by 58 percent over about three years in the Diabetes Prevention Program.

HbA1c reflects about three months, so it is usually rechecked then. Blood pressure and triglycerides often improve earlier.

Not always. Weight medication, blood pressure or lipid treatment is added when lifestyle changes are not enough or your risk is high.

The metabolic programme focuses on blood sugar, lipids, blood pressure and liver values and tracks them as the main outcome.

The cost depends on the tests, programme length and any medication. The full cost is explained in the consultation.
