In short
Insulin resistance is a reduced response of muscle, liver and fat cells to insulin, so the pancreas makes more insulin to keep blood sugar normal. Insulin resistance drives metabolic syndrome, prediabetes, fatty liver and polycystic ovary syndrome. It is recognised from waist, blood fats and blood sugar rather than from an insulin test.
Condition Overview
What is insulin resistance?
Insulin resistance is a state in which the body needs more insulin than normal to move sugar into cells. For years the pancreas compensates with high insulin levels and blood sugar stays normal; when the pancreas no longer keeps up, blood sugar rises into prediabetes and later type 2 diabetes.
What are the signs of insulin resistance?
The signs of insulin resistance are a large waist, high triglycerides, low HDL cholesterol, raised blood pressure and borderline blood sugar, the criteria of metabolic syndrome. Dark velvety skin in the neck folds or armpits (acanthosis nigricans) and skin tags are visible signs.
Is there a test for insulin resistance?
Insulin resistance is recognised from waist, triglycerides, HDL, blood pressure and HbA1c rather than from an insulin level. Fasting insulin and HOMA-IR, a calculation from glucose and insulin, vary widely between laboratories and do not predict the response to treatment, so clinical guidance does not recommend them routinely.
Which conditions are linked with insulin resistance?
Insulin resistance is linked with prediabetes, type 2 diabetes, fatty liver, polycystic ovary syndrome and sleep apnoea. Most women with polycystic ovary syndrome have insulin resistance independent of weight: PCOS weight gain.
Insulin resistance in Dubai and the UAE
How early does raised blood sugar appear in the UAE?
Among 33,327 Emirati men aged 18 to 29, 41.3 percent had raised fasting blood sugar, 4 in 10 before the age of 30 (Endocrinology, Diabetes and Metabolism, 2019).
The national UAE health survey 2024 to 2025 found obesity in 22.4 percent of adults, more than 1 in 5, high blood pressure in 25.9 percent, raised blood sugar in 12.5 percent and too little physical activity in 59.1 percent (MOHAP survey, reported by WAM).
Dr Felix treats within the scope of his Dubai Health Authority licence. When a case needs a specialist view, for example suspected Cushing’s syndrome, a severe eating disorder or weight loss surgery, the decision is made together with an endocrinologist, psychiatrist or bariatric surgeon.
How insulin resistance is treated with weight loss: weight loss with insulin resistance, metabolic weight loss and GLP-1 weight loss treatment.
Your first consultation
Sit down with Dr. Felix, say what you want to know and leave with a clear plan. One physician, from the first question to the follow-up.
When to See a Doctor
- Weight collects around your waist despite a normal BMI.
- Your HbA1c is 5.7 percent or higher, or triglycerides are high.
- You have dark velvety skin patches on the neck or armpits.
- You have polycystic ovary syndrome or fatty liver.
- Thirst, frequent urination and weight loss: tests for diabetes the same week.
- Chest pain or breathlessness on exertion: heart assessment first.
- Snoring with breathing pauses and daytime sleepiness: sleep apnoea assessment.

Why Early Treatment Can Matter
Recognising insulin resistance early catches the years before prediabetes and diabetes, when weight loss and activity reverse it most easily.

Reversible stage
Insulin resistance improves with weight loss and activity.

Rising blood sugar is caught before diabetes.
