In short

Testosterone gel is a daily skin gel and the usual first choice for testosterone replacement therapy (TRT), because it gives normal and relatively stable levels. The gel is applied to the shoulders and upper arms and covered once dry, because skin contact can transfer testosterone to a partner or child. Levels vary from day to day, so two blood samples are taken before a dose change.

Treatment Overview

What is testosterone gel?

Testosterone gel is a daily skin gel and the usual first choice for testosterone replacement therapy, because it gives normal and relatively stable levels and most men prefer it. Testosterone gel treats a confirmed testosterone deficiency; it does not treat tiredness with a normal testosterone. All forms are compared on the testosterone replacement therapy page.

How is testosterone gel used?

Testosterone gel is applied to clean, dry skin at the sites specified for the prescribed product. Measure the dose using its metered pump or sachet, never a household teaspoon. Gels of different strengths, including 1% and 1.62%, are not interchangeable. The amount of testosterone applied is not the amount absorbed into the bloodstream. Wash your hands after applying it, let the area dry and cover it with clothing.

How are testosterone levels checked on gel?

Monitoring and the timing of the blood sample depend on the exact gel. Dose titration may need an early blood test; for example, AndroGel 1.62% specifies checks around days 14 and 28. Follow the prescribed product instructions and your clinician’s schedule. Do not change the dose yourself. Subsequent reviews assess symptoms, testosterone, blood count, blood pressure and relevant prostate risk.

Can testosterone gel pass to other people?

Testosterone gel can pass to a partner or child through skin contact. This is rare. The US Food and Drug Administration (FDA) received 8 reports of young children, aged 9 months to 5 years, with signs of testosterone exposure. That compares with about 1.8 million prescriptions. After these reports, the FDA added a boxed warning, its strongest label warning. The package instructions reduce the risk to a minimum:

  • Wash hands thoroughly after applying the gel.
  • Prevent transfer: keep the application area covered after drying. Before expected direct skin contact, wash the area with soap and water.
  • Cover the application site with clothing once dry.
  • Follow the exact product instructions for application sites and the minimum time before showering or swimming; this differs between gels.

What are the side effects of testosterone gel?

The side effects of testosterone gel are skin irritation at the application site, which is uncommon, and the general effects of testosterone, such as a rising red blood cell count and acne. Erythrocytosis was very low in the Testosterone Trials and the TRAVERSE trial, where testosterone gel was dosed to keep levels in the normal range. The full risk picture is on the testosterone replacement therapy page.

Testosterone gel in Dubai and the UAE

How is testosterone gel stored in Dubai heat?

Testosterone gel is stored at normal room temperature, 20 to 25 °C, and may briefly be between 15 and 30 °C, according to the US prescribing information for AndroGel 1.62%. Outdoor and in-car temperatures in a Dubai summer exceed 30 °C, so the gel stays indoors. Alcohol-based gels are flammable until dry, so fire, flames and smoking are avoided until the gel has dried.

Is testosterone gel legal in the UAE?

Testosterone gel is legal in the UAE only with a prescription from a licensed doctor, dispensed by a licensed pharmacy. The rules for bringing testosterone into the UAE are explained under bringing testosterone into the UAE.

Who Is a Good Candidate?

  • Men with symptoms of low testosterone and a low morning fasting testosterone confirmed on at least two days.
  • Men whose low testosterone comes from the testes or the pituitary gland, established with LH and FSH.
  • Prostate or breast cancer, a haematocrit above 50 percent, severe untreated sleep apnoea or uncontrolled heart failure: treated or excluded first.
  • Doses aimed above the normal range, or a prescription without examination and blood tests: not offered.
  • Young children or a pregnant partner in regular skin contact: an injection may suit better.

Program Structure & Follow-Up

How is testosterone gel treatment followed up?

Testosterone gel treatment is followed up at 2 to 3 months with two testosterone samples, at 3 to 6 months with haematocrit, and at 12 months with a full review, then every 6 to 12 months. Contact between appointments runs through the membership programmes.

Benefits, Limits & Safety

What happens after stopping testosterone gel?

After stopping testosterone gel, testosterone stays low for months while the body restarts its own production, and the original symptoms return. A planned stop is scheduled with blood tests.

Testosterone products can raise blood pressure. Blood pressure is checked before and during treatment, alongside the other monitoring appropriate to the product and your medical history.

What gel offers

Normal and relatively stable testosterone levels with a daily application, preferred by most men.

How you are followed

Testosterone at 2 to 3 months and after each dose change, then every 6 to 12 months. Haematocrit at 3 to 6 months, then yearly. Prostate checks at 3 and 12 months from age 50.

When gel is not used

Prostate or breast cancer, raised haematocrit, severe untreated sleep apnoea, uncontrolled heart failure, or a current wish for children.

Cost & Program Investment

The cost depends on the preparation and the monitoring it needs. Dr. Felix explains the full cost before any prescription. Ongoing care runs through the membership programmes.

Frequently Asked Questions

Use only the application sites specified for your prescribed gel. For AndroGel 1.62%, these are the shoulders and upper arms, not the abdomen. Let it dry and cover the area; wash it before expected direct skin contact.

Testosterone gel usually brings levels into the normal range within a month. Energy, libido and lean body mass change within 3 to 6 months, bone density over about 24 months.

Follow the minimum waiting time in the instructions for your exact gel. The interval differs by product and is not a universal five hours. Before expected direct skin contact with another person, wash the treated area with soap and water.

Testosterone gel is usually the first choice because of stable levels and patient preference. Injections suit men who prefer fewer doses or have young children at home.

The cost of testosterone gel in Dubai depends on the product, the dose and the monitoring. Dr. Felix explains the full cost before any prescription.
