In short

Ozempic is a weekly semaglutide injection licensed for adults with type 2 diabetes, not for weight management. Ozempic lowers blood sugar, reduces cardiovascular events in people with diabetes and heart disease, and often lowers weight. For weight loss without diabetes, the semaglutide product licensed and studied is Wegovy. In Dubai, Ozempic needs a doctor’s prescription.

Treatment Overview

What is Ozempic?

Ozempic is a once-weekly semaglutide injection for type 2 diabetes. Semaglutide acts like GLP-1 (glucagon-like peptide 1), a gut hormone that releases insulin when blood sugar is high, lowers glucagon, slows stomach emptying and signals fullness to the brain. The first two effects lower blood sugar. The last two reduce appetite, which explains the weight loss many patients notice.

What is Ozempic licensed for?

Ozempic is licensed for adults with type 2 diabetes, according to the US prescribing information. The licence covers three goals: better blood sugar control alongside diet and exercise, fewer major cardiovascular events in people with established heart disease, and slower kidney function decline in people with chronic kidney disease. Weight management is not part of the Ozempic licence.

Prescribing Ozempic only for weight is off-label use. Off-label use is legal, but the prescribing doctor has to justify why a licensed alternative is not the better choice.

Should you choose Ozempic or Wegovy?

Ozempic and Wegovy contain the same active ingredient but differ in licence and dose. The table compares the two semaglutide injections.

Ozempic and Wegovy compared (injection forms)
Ozempic Wegovy
Active ingredient Semaglutide Semaglutide
Licensed for Type 2 diabetes Weight management
Usual top dose 2 mg once weekly 2.4 mg once weekly
Tested for weight loss in people without diabetes No Yes, in the STEP trials

For weight loss without diabetes, Wegovy is the semaglutide product with trial evidence and a licence for that purpose. For adults with type 2 diabetes and excess weight, Ozempic can treat both problems together.

How is the Ozempic dose increased?

The Ozempic dose starts at 0.25 mg once a week and rises in steps of at least four weeks, up to 2 mg, according to the US prescribing information. The 0.25 mg starting dose mainly helps the stomach adapt and has little effect on blood sugar. The table lists the steps and the minimum time on each.

Ozempic dose steps (once weekly)
Step Weekly dose Minimum time
Start 0.25 mg 4 weeks
Second step 0.5 mg 4 weeks
If more blood sugar control is needed 1 mg 4 weeks
Maximum 2 mg Ongoing

A dose step is taken only when blood sugar control requires it and the previous step is well tolerated.

Does Ozempic protect the heart and kidneys?

Semaglutide 0.5 or 1 mg reduced major cardiovascular events from 8.9 to 6.6 percent in the SUSTAIN-6 trial. SUSTAIN-6 followed 3,297 adults with type 2 diabetes and high cardiovascular risk for a median of two years. The benefit came mainly from fewer non-fatal strokes (1.6 versus 2.7 percent).

In a separate trial of 3,533 adults with type 2 diabetes and chronic kidney disease, semaglutide 1 mg weekly lowered the combined risk of kidney failure, major kidney function loss and kidney or cardiovascular death by about a quarter over 3.4 years. The US licence now includes this kidney benefit.

Can Ozempic be prescribed in Dubai for weight loss?

In Dubai, Ozempic needs a doctor’s prescription, and use for weight alone is off-label. Dr. Felix Lucian Happich prescribes Ozempic mainly for adults with type 2 diabetes. For weight loss without diabetes, the consultation usually points to Wegovy or another licensed option. Local details follow in the section Ozempic in Dubai and the UAE.

How should Ozempic pens be stored in Dubai heat?

Unused Ozempic pens belong in the refrigerator at 2 to 8 °C, according to the US prescribing information. After the first injection, a pen can be kept for up to 56 days at 15 to 30 °C or in the refrigerator. Dubai summer temperatures, for example in a parked car, exceed 30 °C, so a pen exposed to that heat is no longer reliable.

Can Ozempic be used during Ramadan?

Ozempic can usually be continued during Ramadan without a dose change, but should not be started in the weeks just before the fast. Clinical guidance for people with diabetes supports continuing GLP-1-based medicines such as semaglutide during Ramadan, because nausea is most common in the first weeks of treatment.

People taking Ozempic together with insulin or sulfonylureas need an adjusted plan before Ramadan, because fasting raises the risk of low blood sugar. Long fasting hours without fluids also raise the risk of dehydration.

Your first consultation

Sit down with Dr. Felix, say what you want to know and leave with a clear plan. One physician, from the first question to the follow-up.

Who Is a Good Candidate?

  • Adults with type 2 diabetes who need better blood sugar control, especially with excess weight.
  • Adults with type 2 diabetes and established heart disease, where semaglutide reduced major cardiovascular events.
  • Adults with type 2 diabetes and chronic kidney disease, where semaglutide slowed kidney function loss.
  • Adults who want a clear comparison of Ozempic, Wegovy and other options before deciding.
  • Personal or family history of medullary thyroid cancer, or multiple endocrine neoplasia type 2 (MEN 2): Ozempic is not used.
  • Pregnancy, plans to conceive soon, or breastfeeding: Ozempic is not used.
  • Severe inflammatory bowel disease or gastroparesis: GLP-1-based medicines are usually avoided.
  • Weight loss without diabetes as the only goal: Wegovy is usually the appropriate licensed choice.
  • Requests for a prescription without examination, or pens from unregulated sources: not offered.

What Happens During the Consultation

  • 1

    Reason for treatment

Dr. Felix clarifies whether the goal is blood sugar, weight or both, which treatments were tried before, and whether pregnancy is planned.

  • 2

    Blood tests and examination

The assessment covers HbA1c, fasting glucose, kidney and liver function, lipids, blood pressure and weight, plus eye health in people with diabetic retinopathy.

  • 3

    Medicines and history

All current medicines are reviewed. Insulin or sulfonylureas are usually reduced when Ozempic starts, because the combination can cause low blood sugar.

  • 4

    Decision and plan

You receive a clear recommendation for Ozempic, Wegovy, another medicine or no medicine, with the reason, the dose plan and the review dates.

Program Structure & Follow-Up

How is Ozempic treatment monitored?

Ozempic treatment is monitored every 4 to 6 weeks while the dose rises, then about every 2 to 3 months. The reviews cover the following points:

  • Blood sugar: HbA1c and home glucose readings.
  • Low blood sugar: daily self-monitoring in the first weeks for people on insulin or sulfonylureas, with dose reductions as needed.
  • Eyes: slower dose steps and eye checks in people with diabetic retinopathy.
  • General health: weight, blood pressure, kidney function and side effects.

Between appointments, patients can contact the practice through the membership programmes.

How is muscle protected during Ozempic treatment?

Muscle is protected through enough protein, strength training and body composition checks. A systematic review of 35 studies found that about 28 percent of the weight lost with GLP-1-based medicines came from muscle-related measures.

What happens after stopping Ozempic?

After stopping Ozempic, blood sugar usually rises again and appetite returns. In people using semaglutide for weight, most of the lost weight returns within a year of stopping. Diabetes treatment is therefore planned long term, and any change is made with a replacement plan.

Benefits, Limits & Safety

What are the side effects of Ozempic?

The most common side effects of Ozempic are nausea, vomiting, diarrhoea and constipation, usually worst after a dose increase. Across GLP-1-based medicines, 25 to 40 percent of patients experience nausea and about 10 to 20 percent vomiting, diarrhoea or constipation. Most symptoms are mild to moderate and improve with time.

Serious but less common problems include gallbladder disease, pancreatitis and bowel obstruction. On its own, Ozempic rarely causes low blood sugar; with insulin or sulfonylureas the risk rises.

Rapid improvement in blood sugar can temporarily worsen diabetic eye disease. People with retinopathy therefore increase the dose more slowly and have their eyes monitored.

How can nausea on Ozempic be reduced?

Nausea on Ozempic often improves with smaller meals, slower eating and a delayed dose step. Clinical guidance lists these measures:

  • Eat smaller, more frequent meals.
  • Stop eating at the first sign of fullness.
  • Limit fatty, greasy and spicy food.
  • Delay the next dose increase if nausea persists.

When should you seek urgent medical help on Ozempic?

Seek urgent medical help on Ozempic for severe abdominal pain, persistent vomiting, signs of gallbladder problems or sudden vision changes:

  • Severe abdominal pain that spreads to the back.
  • Persistent vomiting that prevents keeping fluids down.
  • Pain under the right ribs, fever, or yellow skin or eyes.
  • Sudden vision changes.
  • Symptoms of low blood sugar such as shaking, sweating or confusion, especially with insulin or sulfonylureas.

In the UAE, call 998 for an ambulance in an emergency.

What should other doctors know about Ozempic?

Other doctors need to know that Ozempic slows stomach emptying. Before a planned operation with anaesthesia, Ozempic may need to be paused because of the risk of aspiration.

Why are compounded semaglutide and prescriptions without examination not offered?

Compounded semaglutide is not offered because compounded copies do not go through regulatory review for safety, quality or effectiveness, and safety problems have been reported. Prescriptions without an examination are not offered, because blood sugar, kidney function and medicines need to be checked first.

Only buy these medicines from licensed pharmacies. What the Emirates Drug Establishment did about unapproved peptide sellers is described on the prescription weight loss page.

What the evidence shows

Better blood sugar control in type 2 diabetes. In SUSTAIN-6, major cardiovascular events fell from 8.9 to 6.6 percent. Weight loss is common but is not the licensed purpose.

How you are followed

Reviews every 4 to 6 weeks during dose steps, then about every 2 to 3 months, with HbA1c, kidney function, weight, blood pressure and glucose self-monitoring when needed.

When Ozempic is not used

Pregnancy, breastfeeding, personal or family history of medullary thyroid cancer or MEN 2. Usually avoided with severe inflammatory bowel disease or gastroparesis.

Cost & Program Investment

What Ozempic treatment costs depends on the dose and how long you stay on it. Dr. Felix explains the figure before any prescription.

Frequently Asked Questions

Ozempic is licensed for type 2 diabetes, not for weight management. Weight loss is a common effect of semaglutide. The semaglutide product licensed and tested for weight loss is Wegovy, at doses up to 2.4 mg weekly.

Ozempic and Wegovy both contain semaglutide. Ozempic is licensed for type 2 diabetes and goes up to 2 mg weekly. Wegovy is licensed for weight management and goes up to 2.4 mg weekly. The weight loss evidence comes from the Wegovy trials.

Ozempic for weight without diabetes is off-label use and needs a clear medical reason. In most cases, Wegovy is the appropriate choice because Wegovy is licensed and tested for weight management. The decision follows an examination.

Ozempic starts lowering blood sugar within the first weeks once the dose passes the 0.25 mg starting step. HbA1c reflects the full effect after about three months.

A missed Ozempic dose is taken within 5 days, according to the US prescribing information. After more than 5 days, the missed dose is skipped and the next dose is taken on the usual day.

Ozempic alone rarely causes low blood sugar. Combined with insulin or sulfonylureas, low blood sugar becomes more likely, so these medicines are often reduced when Ozempic starts and glucose is checked daily during the first weeks.

Unused Ozempic pens are stored in the refrigerator at 2 to 8 °C. After first use, a pen can be kept for up to 56 days at 15 to 30 °C, according to the US prescribing information. Heat above 30 °C should be avoided.

Ozempic can usually be continued during Ramadan without a dose change. Starting Ozempic in the weeks just before Ramadan is discouraged because nausea is most common early in treatment. Insulin and sulfonylureas often need adjusting for the fast.

Blood tests are needed before and during Ozempic treatment. HbA1c, kidney function, liver function and lipids show whether treatment works and whether other medicines need adjusting.

After stopping Ozempic, blood sugar and appetite usually rise again, and weight tends to return. Any change to diabetes treatment is therefore planned together with a replacement.

The cost of Ozempic in Dubai depends on the dose and the length of treatment. Dr. Felix explains the full cost in the consultation before any prescription.
