TRT and Fertility in Dubai
TRT and fertility planning is simple with Dr Felix's personal care. There is no testosterone before your plans for a family are clear, no guessing how long sperm takes to recover, and every option is weighed against your fertility.
Your first visit includes a full check-up and blood work with testosterone, LH and FSH. When a baby is part of your plans now or later, your whole plan is built around that.

Fertility asked first
Testosterone therapy lowers sperm production. Dr Felix asks about your plans for children before anything is prescribed.
The cause decides the route
Low testosterone from low brain signals responds to treatments that restart your own testosterone and sperm.
One doctor for the whole plan
This is a personal practice. Dr Felix Happich, German-trained and licensed by the Dubai Health Authority, stays your doctor from the first test to the follow-up.

What does TRT and fertility planning with Dr Felix include?
TRT and fertility planning with Dr Felix includes 6 parts. The plan starts with the cause of the low testosterone and your timeline for a family.
- Testosterone: measured fasting between 8 and 10 in the morning, on two separate days.
- LH and FSH: luteinising hormone and follicle-stimulating hormone show whether the testes or the brain signals are the cause.
- Further tests: prolactin and iron when the brain signals are low.
- Semen analysis: sperm count and movement when you plan children.
- Weight and sleep: both lower testosterone and both respond to treatment.
- Fertility specialist: involved when hormone treatment for sperm production is needed.

Does TRT cause infertility?
Yes, TRT (testosterone replacement therapy) lowers sperm production, often to zero. Testosterone from outside the body switches off LH and FSH, the brain signals that drive sperm production in the testes (Endocrine Society guideline, 2018).
The guideline advises against testosterone therapy in men who plan a pregnancy in the near future. The testes also get smaller on testosterone, a visible sign of suppressed sperm production.
How long does sperm take to recover after stopping testosterone?
Sperm production usually recovers within months after stopping testosterone. In an analysis of 1,549 healthy men who took testosterone in contraception studies, sperm returned in all men within 2 years. Sperm counts of at least 20 million per mL came back in about two thirds (67 percent) within 6 months, in 9 out of 10 (90 percent) within 12 months, and in everyone within 24 months (The Lancet, 2006).
These men used testosterone for a limited time. Recovery takes longer after years of use or after high doses.
Which options protect fertility with low testosterone?
Hormone treatment that restarts the testes protects fertility when low testosterone comes from low brain signals. hCG (human chorionic gonadotropin), a hormone that acts like LH, makes the testes produce their own testosterone and sperm, and FSH is added when the sperm count stays low.
With this treatment, sperm appears in up to 90 percent of men with low brain signals. In one study of 75 men, the first sperm appeared after a median of 7.1 months, and pregnancy took longer. This treatment is planned together with a fertility specialist.
What else raises testosterone without affecting fertility?
Weight loss raises testosterone without any effect on sperm production. A review of 24 studies found that weight loss raises testosterone in men with obesity (European Journal of Endocrinology, 2013).
- Weight: excess weight lowers testosterone, and weight loss raises it.
- Test timing: testosterone is highest in the morning and lower after food.
- Illness: testing waits until an acute illness is over, because the drop is temporary.
- Medicines: long-term opioids and high-dose steroid tablets lower testosterone and are reviewed.

More questions about TRT and fertility
What does TRT and fertility planning cost?
The cost depends on the blood tests and semen analysis you need and whether treatment becomes part of your plan. You get the full cost in the consultation before anything starts.
I am on TRT and want a baby. What now?
Testosterone is stopped with a plan, and sperm production is checked over the following months. If sperm does not return, hormone treatment with a fertility specialist is the next step.
Does low testosterone mean I am infertile?
No, not necessarily. Testosterone in the blood and sperm production are linked, but only a semen analysis shows your fertility.
Can I start TRT after we have had our children?
Yes, TRT is possible after your family is complete, if low testosterone is confirmed and nothing speaks against it. Sperm production stays suppressed while you take it.
