In short

Male hormonal fatigue describes persistent tiredness in men for which hormones are suspected. A cause is found in about two thirds of people with chronic fatigue, most often sleep apnoea, thyroid disease, anaemia or iron deficiency, depression, alcohol or medicines. Low testosterone is one cause among many: in the Testosterone Trials, testosterone did not improve energy in the trial designed to measure it.

Condition Overview

What is male hormonal fatigue?

Male hormonal fatigue is persistent tiredness in a man in whom a hormonal cause is suspected. Fatigue covers three different complaints: difficulty starting activity, tiring quickly, and poor concentration and memory. Some men who say “tired” mean sleepiness, which points to a sleep disorder. Separating these complaints is the first step, because each leads to different causes.

What causes fatigue in men?

Fatigue lasting more than a month is caused mainly by chronic medical conditions, low mood or anxiety, medicines and substances. The table lists common causes and the clue that points to each.

Common causes of persistent fatigue in men
Cause Typical clue
Obstructive sleep apnoea Snoring, witnessed breathing pauses, unrefreshing sleep
Anaemia or iron deficiency Breathlessness on effort, dizziness, poor recovery
Thyroid disease Cold or heat intolerance, weight change, heart rate change
Diabetes and kidney or liver disease Thirst, weight change, abnormal routine blood tests
Depression or anxiety Loss of interest and pleasure, constant worry
Medicines Timing that matches a new beta blocker, antidepressant, sedating antihistamine, opioid or gabapentin
Alcohol and other substances Regular drinking, cannabis, stimulants
Low testosterone Low libido and fewer morning erections alongside the fatigue

Several causes often occur together, which is a common reason tiredness persists after one of them is treated.

Does low testosterone cause fatigue?

Low testosterone causes fatigue in some men, but testosterone treatment did not improve energy in the Testosterone Trials vitality study, which treated men over 65 with confirmed low testosterone for one year (Testosterone Trials). Sexual desire, mood, anaemia and bone density improved instead. A man whose main complaint is tiredness therefore rarely benefits from testosterone alone. Low testosterone itself is explained on the low testosterone page.

Why is sleep assessed first in fatigue?

Sleep is assessed first because sleep apnoea and short sleep are common, treatable causes of fatigue, and fatigue that improves after sleep suggests a primary sleep disorder. Snoring, witnessed pauses in breathing and unrefreshing sleep are therefore part of every fatigue assessment.

Which tests are done for fatigue?

The basic tests for fatigue lasting more than a month are a full blood count, glucose, electrolytes, calcium, kidney and liver function and thyroid-stimulating hormone (TSH). Ferritin, morning testosterone and a sleep study are added when the history points to them. Large panels without a clear reason help little: in a study of 100 adults with fatigue, laboratory tests clarified the cause in only 5 percent, and unfocused testing produces false-positive results.

How long does fatigue last before it is investigated?

Fatigue is classified by duration as acute up to one month, subacute from one to six months and chronic beyond six months. Acute fatigue after an infection or a stressful event usually needs little testing. Fatigue beyond one month without an obvious reason is assessed, and without a cause it is reassessed after one to three months.

When to See a Doctor

  • Tiredness has lasted more than a month and does not lift after rest or a holiday.
  • Fatigue comes with reduced libido, low mood or loss of strength.
  • Snoring, gasping at night or witnessed breathing pauses are present.
  • Blood tests were called normal without an explanation of what was tested.
  • Unintentional weight loss, fever, night sweats or swollen glands: prompt investigation.
  • Breathlessness, chest discomfort or rapidly worsening fatigue: urgent medical assessment.
  • Blood in the stool or urine, or marked paleness: investigation before any hormone question.

How Dr. Felix Assesses This

  • 1

    Characterising the fatigue

Dr. Felix separates sleepiness, exhaustion and loss of motivation, and asks when the fatigue began, how it varies and what it prevents.

  • 2

    Sleep, mood, substances and medicines

Sleep hours, snoring, shift work and travel, mood and anxiety, alcohol and other substances, and every medicine and supplement are reviewed.

  • 3

    Examination and focused tests

The examination covers thyroid, lymph nodes, heart, lungs and muscles. Blood tests start with blood count, glucose, electrolytes, calcium, kidney and liver function and TSH, with further tests where the history points.

  • 4

    Diagnosis and plan

Identified causes are treated in order, and fatigue without a cause is reassessed after one to three months.

Treatment Options

Why Early Treatment Can Matter

Early assessment of fatigue finds correctable causes such as anaemia, thyroid disease and sleep apnoea before months of tiredness affect training, diet and mood.

Fatigue that persists after the first cause is treated leads to a repeat of the initial assessment, because a second cause is common.

A cause in most cases

A cause is found in about two thirds of people with chronic fatigue.

Sleep apnoea matters

Untreated sleep apnoea affects blood pressure and daytime safety as well as energy.

Focused testing

Tests chosen from the history avoid false-positive results from large panels.

Cost & Consultation Investment

The cost of a fatigue assessment covers consultation time with Dr. Felix, the examination, the choice of tests and the written plan. Laboratory tests and any sleep study are separate. Dr. Felix explains the full cost before anything is arranged.

Frequently Asked Questions

Tiredness is caused by low testosterone less often than by sleep apnoea, anaemia, thyroid disease, depression, alcohol or medicines. Low libido and fewer morning erections alongside the tiredness make low testosterone more likely.

Testosterone did not improve energy in the Testosterone Trials vitality study after one year, although sexual desire, mood, anaemia and bone density improved.

Snoring and witnessed breathing pauses point to obstructive sleep apnoea, a common and treatable cause of fatigue.

Yes, iron deficiency without anaemia is a recognised cause of chronic fatigue, and ferritin is checked when the history points to it.

Benzodiazepines, antidepressants, muscle relaxants, sedating antihistamines, beta blockers, opioids and gabapentin are associated with fatigue.

Large panels without a clear reason clarified the cause of fatigue in only 5 percent of adults in one study and produce false-positive results that lead to unnecessary tests.

Fatigue without a cause is reassessed after one to three months. Depressive symptoms, cognitive behavioural therapy and graded exercise are options for fatigue that remains unexplained.

The cost of a fatigue assessment in Dubai depends on the tests and whether a sleep study is needed. Dr. Felix explains the full cost before anything is arranged.
