What does low libido treatment after menopause include?

Treatment with Dr Felix starts with what bothers you. Your plan is built on your symptoms, your health and your relationship.

  • Desire and arousal: what changed and how much it bothers you.
  • Menopause symptoms: hot flushes and poor sleep lower desire.
  • Vaginal health: dryness and pain checked.
  • Medications: antidepressants and other drugs that lower desire.
  • Mood and stress: talked about openly.
  • Your relationship: with a referral to a sex therapist when helpful.

Why does libido drop after menopause?

Low desire after menopause usually has more than one cause. Dryness and pain, poor sleep, mood, medications and the relationship all play a part, next to hormones.

Low desire only needs treatment when it bothers you. Many women find that treating pain or sleep first brings desire back.

Does testosterone help women?

For some women, yes. A review of 36 trials with 8,480 women found that testosterone improved desire, arousal, pleasure and satisfying sexual events after menopause (Lancet Diabetes & Endocrinology, 2019).

The international consensus supports testosterone only for low desire that causes distress, at doses that keep levels in the normal female range (J Clin Endocrinol Metab, 2019). Compounded creams are not used here.

Which treatments are used?

Treatment follows a clear order. Each step is tried and reviewed before the next one.

  • Vaginal oestrogen: a low dose for dryness and pain.
  • HRT patch or gel: when hot flushes and poor sleep play a part. Tablets lower free testosterone, patches do not.
  • Medication review: changing drugs that lower desire, where possible.
  • Sex therapy: couples counselling and sex therapy help many women.
  • Testosterone: considered after the other steps, at female doses, with blood tests.

What else helps desire?

Everyday things affect desire more than many women expect. Better sleep and less stress often help as much as medication.

  • Sleep: treated when it is poor.
  • Exercise: improves mood and energy.
  • Alcohol: less is better.
  • Time together: planned, not left to chance.
  • Lubricants: help when dryness makes sex uncomfortable.
