Regain is biology

After weight loss, the body burns less and hunger rises. Your plan is built for that, not for another short diet.

02

A plan for after the medicine

Many people regain after stopping Wegovy or Mounjaro. Dr Felix plans the dose, how long to take it and what happens if you stop.

What does a weight regain plan include?

A weight regain plan with Dr Felix starts with what happened last time. Your plan is built on your past attempts, your blood work and your body composition.

  • History: every diet and medicine you tried, what worked and why it stopped.
  • Blood work: glucose, HbA1c, thyroid, liver and cholesterol, to check nothing new is driving the regain.
  • Body composition: how much muscle earlier diets cost you.
  • Protein and strength targets: to rebuild and keep muscle.
  • Medication: only with a medical reason, with a long-term plan for the dose.
  • Follow-up: fixed review dates, also once the weight is off.

How fast does weight come back after stopping medication?

Weight usually comes back after weight loss medication is stopped. A review of 37 studies with over 9,300 people found an average regain of 0.4 kg per month after stopping, with a return to the starting weight within about two years.

0.4 kgregained per month on average after stopping weight loss medication.Review of 37 studies

In a trial of 378 people who had lost weight on tirzepatide (Mounjaro), 67 percent of those switched to placebo regained at least half of the loss within a year. On the full dose, 11 percent did, and on a lower dose 25 percent.

Should I restart my weight loss injection?

Restarting makes sense if the medicine worked for you and you tolerated it. When a weight loss medicine brings more than 5 percent loss, long-term use is recommended, because obesity is a chronic condition and stopping leads to regain.

Once the maintenance dose is reached, it is usually kept, because a lower dose leads to more regain. If the response is insufficient, the prescriber reviews dose, duration, tolerability and the product-specific continuation criteria before changing treatment.

How do I keep muscle this time?

Muscle is what repeated diets often cost. In a review of 35 studies, about 28 percent of the weight lost on GLP-1 medicines came from muscle-based tissue, and muscle loss also happens with diets alone.

Protein at every meal and a mix of strength and cardio training limit this loss. Your plan sets both targets and checks body composition at follow-up, so you see whether the weight lost is fat.

What else helps keep weight off?

Keeping weight off works better with a clear plan than with willpower. A plateau on medication does not mean the medicine stopped working. It means further loss needs another step.

  • Structure: regular reviews and set targets, like in the large diabetes prevention programmes, can help people keep weight off long term.
  • Activity: daily movement plus strength training.
  • Blood work: a new cause, such as a thyroid problem, is ruled out.
  • Surgery: an option for people who meet the criteria and do not respond enough to medication.

More questions about weight regain

What does weight regain treatment cost?

The cost of weight regain treatment depends on the blood tests you need and whether medication becomes part of your plan. You get the full cost in the consultation before anything starts.

I stopped Mounjaro months ago. Can I restart?

Often yes, after a medical check. Dr Felix decides the starting dose based on how long you have been off it and how well you tolerated it before.

Do I have to take the medicine forever?

For many people, long-term use is what keeps the weight off, because regain is expected after stopping. Dr Felix plans the duration with you and what happens if you want to stop.

My weight loss stopped on the injection. Is it still working?

Yes. When a weight loss medicine reaches its full effect, weight levels off. Further loss needs another step, such as more activity, a diet change or a different medicine.
