What does a PCOS weight loss plan include?

A PCOS weight loss plan with Dr Felix starts with measurements, not guesses. Your plan is built on your blood work, your body composition and your pregnancy plans.

  • Blood work: blood sugar, HbA1c, cholesterol, liver and thyroid.
  • Body composition and waist: not only the number on the scale.
  • Nutrition and training targets: set around your routine and your food preferences.
  • Medication: only with a medical reason, with contraception and stop dates planned.
  • Follow-up: fixed review dates with repeat blood tests.

Does losing weight really help PCOS?

Weight loss is the first treatment for most women with PCOS and excess weight. In a UK study of 75 women with PCOS, ovulation returned within a year in 19 percent of those who did not lose weight and in more than half of those who did. Every percent of weight lost raised the chance further (BAMBINI analysis, Human Reproduction 2026).

90 kg body weight×5 percent→4.5 kg

A loss of 5 to 10 percent is the range linked to returning ovulation in women with PCOS. The response differs between women, and some do not get regular cycles despite a similar loss.

How do I lose PCOS belly fat?

PCOS belly fat is fat stored around the waist, and in PCOS it is closely linked to insulin resistance, which is common whatever your weight. Belly fat goes down with overall weight loss, and no single diet has been shown to work better for women with PCOS.

Exercise can improve ovulation and insulin sensitivity in PCOS, even when the weight loss is modest. That is why your waist is measured at every review, not only your weight.

Can I get weight loss medication with PCOS?

Weight loss injections such as semaglutide (Wegovy) and tirzepatide (Mounjaro) lead to similar weight loss in women with and without PCOS. Medication is considered at a BMI of 30 or more, or 27 or more with a condition such as high blood pressure, alongside nutrition and activity support, after an individual assessment of prior efforts, clinical needs and the applicable product information.

The prescriber reviews response and tolerability using the continuation criteria for the particular medicine; there is no single 12-week stopping rule for every product. A 2026 review of 11 randomised trials in women with PCOS found a modest extra drop in BMI with GLP-1 medicines, while the effect on cycles and hormones is still unclear (Forslund et al., European Journal of Endocrinology). The injections treat weight. They are not a fertility treatment.

What if I want to get pregnant?

Weight loss injections are not used in pregnancy, so the timing follows your plans. Semaglutide is stopped at least two months and tirzepatide at least one month before trying to conceive (EMA, Wegovy and Mounjaro product information).

Tirzepatide can make the pill less reliable in women with excess weight. The UK medicines regulator MHRA advises a non-oral method or condoms for four weeks after starting and after every dose increase (Pharmaceutical Journal, June 2025).

  1. Not planning a babyMedication is an option, with reliable contraception
  2. Planning a babyStop semaglutide 2 months or tirzepatide 1 month before trying
  3. Trying to conceiveDiet and exercise, fertility care with your gynaecologist

What else helps with PCOS weight loss?

No single diet works best for PCOS. The 2023 international PCOS guideline found no diet or exercise plan with benefits over others in women with PCOS, so the plan that works is the one you can keep.

  • Low-carb: fine if it suits you. In a 12-week study of 28 women with PCOS, a lower-carb and a standard diet gave the same weight loss and cycle improvements.
  • Exercise: linked to modest weight loss and better ovulation and insulin sensitivity in PCOS.
  • Sleep: sleep apnoea is common in PCOS and worsens insulin resistance. Loud snoring and daytime tiredness are worth mentioning.
  • Metformin: an older tablet. The guideline suggests it at a BMI of 25 or more to improve weight and blood values, with low-certainty evidence.
  • Inositol: widely sold for PCOS. A 2024 review of 30 trials found the evidence limited and inconclusive.
  • Surgery: an option at a BMI of 35 or more. In the BAMBINI trial, women with a sleeve gastrectomy had 2.5 times more spontaneous ovulations over one year than women on medical care.

More questions about PCOS weight loss

What does PCOS weight loss treatment cost?

The cost of PCOS weight loss treatment depends on the blood tests you need and whether medication becomes part of your plan. You get the full cost in the consultation before anything starts.

Do I still need my gynaecologist?

Yes. Your gynaecologist stays in charge of your cycle, contraception choices and fertility. Dr Felix handles weight and metabolism.

I take the pill. Can I use weight loss injections?

Yes, with a plan. Tirzepatide can make the pill less reliable, so a non-oral method or condoms are advised for four weeks after starting and after each dose increase.

I want a baby next year. Is it worth starting now?

Losing weight before fertility treatment can help women with PCOS. In a US study, women who lost about 6.5 percent over 16 weeks first had a live birth rate of 25 percent, compared with 10 percent with immediate fertility treatment.
