In short

Weight maintenance is the planned phase after weight loss, because the body lowers its energy use and increases hunger to regain the lost weight. After stopping obesity medication, people regain on average 0.4 kg per month. A maintenance plan covers medication, protein, activity and early warning signs.

Treatment Overview

Why does weight come back?

Weight comes back because the body treats the previous weight as its target. After weight loss it releases hormones that increase hunger and burns less energy. Clinical guidance describes this as a biological “set point”. Approaches that rely only on willpower often fail for this reason.

How much weight is regained?

A meta-analysis of 37 studies found an average regain of 0.4 kg per month after stopping obesity medication, with a projected return to baseline within two years.

In the STEP 1 extension, adults who stopped semaglutide had lost 17.3 percent on average and regained 11.6 percentage points within a year, leaving a net loss of 5.6 percent.

Should you continue, reduce or stop medication?

The decision is made deliberately, because each option has a measured consequence. A 52-week trial of tirzepatide maintenance in 378 adults showed the differences below.

Tirzepatide maintenance trial, 52 weeks
Maintenance option Weight below baseline at 52 weeks
Full tolerated dose 21.9 percent
Reduced dose (5 mg) 16.6 percent
Placebo 9.9 percent

Regain of at least half the lost weight occurred in 11 percent of participants on the full dose and 25 percent on the reduced dose.

Clinical guidance keeps the established maintenance dose unless weight falls below the goal. Stopping remains a valid choice when the expected regain is understood and planned for.

What helps keep weight off?

Clinical guidance lists frequent self-weighing, regular physical activity, a reduced-calorie diet and ongoing programme attendance as traits of people who keep weight off.

  • Activity: about 300 minutes of moderate exercise per week counters the lower energy use.
  • Strength training: helps preserve muscle.
  • Protein: 1 to 2 g per kg of ideal body weight daily.
  • Weighing: regular self-weighing detects regain early.

Weight maintenance in Dubai and the UAE

About 22 percent of adults in the UAE live with obesity, according to the national health survey. The survey figures are on the medical weight loss page.

Maintenance targets are set against the threshold that applies to you, which is lower for several ethnic backgrounds. The NICE thresholds.

How is Ramadan handled in maintenance?

Ramadan is planned at a review before the fast, including meal timing, protein and any medication. Clinical guidance on religious fasting advises a separate plan for insulin or sulfonylureas because of the risk of low blood sugar.

Weight maintenance protects the result against weight regain. When the weight has already come back: weight regain treatment.

Who Is a Good Candidate?

  • Adults who have reached their target weight and want to keep it.
  • Adults who lost weight on a GLP-1 medicine elsewhere without a maintenance plan.
  • Adults who have regained weight after previous successful attempts.
  • Still in the active weight loss phase: the medical weight loss programme is the starting point.
  • Pregnancy or breastfeeding: weight targets are set differently.
  • Signs of an eating disorder: specialist care comes first.

What Happens During the Consultation

  • 1

    Where you are

Starting weight, lowest weight, current weight, how the weight was lost and what your appetite is doing now.

  • 2

    What you kept

Body composition to see how much muscle was lost, plus blood pressure and blood tests.

  • 3

    Medication decision

Continue, reduce or stop, with the expected consequence of each option explained.

  • 4

    Maintenance plan

Protein target, training, a weight range that triggers a review, and the review dates.

Program Structure & Follow-Up

How often are reviews?

Reviews take place about every 2 to 3 months in the first year, then at longer agreed intervals once weight is stable. A two kilogram drift does not have to wait for the next review: the membership programmes.

What is the early warning range?

You leave with a weight range rather than a single number. Crossing the top of that range triggers an earlier review, so a small regain is handled before it grows.

Why do protein and training matter?

Muscle supports energy use and is often lost during fast weight loss. Clinical guidance recommends resistance training and enough protein to counter the metabolic changes after weight loss. Body composition is measured at each review.

Benefits, Limits & Safety

What does a maintenance plan achieve?

A maintenance plan detects regain early and turns medication decisions into informed choices. Protecting muscle makes the result easier to hold.

What are the limits?

Maintenance takes ongoing effort. Some regain is common even with a good plan, and stopping medication usually leads to some regain. The goal is to hold most of the result.

What safety points matter?

  • After stopping a GLP-1 medicine: appetite returns over weeks; this is expected and planned for.
  • Diabetes and blood pressure medicines: may need adjusting when weight changes.
  • Very low intake: not used to force weight down, because it costs muscle.

What good maintenance looks like

Holding most of the loss, with stable body composition and blood values, and regain caught early.

How you are followed

About every 2 to 3 months in the first year, then at agreed intervals, with a weight range that triggers an earlier review.

When this route is not right

During active weight loss, pregnancy or breastfeeding. Eating disorders need specialist care first.

Cost & Program Investment

What maintenance costs depends on how long you stay in the programme and whether medication continues. You hear the figure in the consultation.

Frequently Asked Questions

After weight loss the body burns less energy and increases hunger hormones. Clinical guidance calls this a biological set point.

Not necessarily. Stopping usually leads to some regain, so the decision is made with the expected consequence explained.

Yes, but in a tirzepatide trial a reduced dose held 16.6 percent weight loss against 21.9 percent on the full dose.

Clinical guidance suggests about 300 minutes of moderate activity per week, plus strength training.

Clinical guidance advises 1 to 2 g per kg of ideal body weight daily. Your target is given in grams.

Yes. The plan starts from your current weight and deals with regain while it is small.

Yes. Your treatment so far is reviewed and then continued or changed.

The cost depends on programme length and whether medication continues. The full cost is explained in the consultation.
