In short

Four things get done properly here that executive packages routinely do badly.

  • Blood pressure. Confirmed outside the clinic before any diagnosis is made, not from one rushed reading.
  • Alcohol. Screened for every year, with a countable question rather than a polite one.
  • Mood and anxiety. Screened with validated tools, then properly assessed if positive.
  • Vaccinations. Checked against the adult schedule, which almost no executive package does.

Tests that are not recommended for you are named and left out, including routine thyroid and vitamin D in people with no risk factors. In the UAE there is a further reason to do this properly: the residence visa medical is a communicable disease screen, not a health check.

Treatment Overview

Why is blood pressure the most commonly mishandled measurement?

Because a single clinic reading in a rushed appointment is exactly the situation most likely to mislead, and it misleads in both directions. Measurement outside the clinic is recommended to confirm a diagnosis of high blood pressure before any treatment is started. The definition of high blood pressure itself depends on how it was measured.

The table gives the threshold that applies to each measurement method, which is why a reading cannot be interpreted without knowing how it was taken.

Thresholds for high blood pressure by measurement method
How it was measured What counts as high
Office readings An average of 130/80 mmHg or more, from at least two measurements per visit on at least two visits spaced over a week or more
24 hour ambulatory monitoring A 24 hour average of 125/75 mmHg or more, or a daytime average of 130/80 or more
Home self-measurement An average of 130/80 mmHg or more, from at least 12 readings taken over at least a week, morning and evening

Two patterns only become visible once measurement moves outside the clinic. White coat hypertension means the clinic reading is high and the out-of-clinic average is normal, and it is a reason not to start or intensify treatment. Masked hypertension is the reverse, a normal clinic reading with a raised average at home, and it needs treating promptly because the risk tracks the out-of-clinic number.

How often it is measured depends on age and risk. The table gives the screening interval.

How often blood pressure is screened
Who Interval
Adults from 40 Yearly
Adults 18 to 39 with normal readings and no risk factors Every three to five years
Higher risk at any age, including readings of 130 to 139 over 80 to 89, overweight or obesity Yearly

Around a third of adults in the UAE have high blood pressure and fewer than a third of them know it. The UAE blood pressure figures.

Why is alcohol asked about specifically?

Because annual screening for unhealthy alcohol use is recommended for every adult, and because a vague question gets a vague answer. In this age group it is one of the highest yield questions in the whole appointment.

The screening question is concrete and countable: do you sometimes drink alcohol, and if so, how many times in the past year have you had five or more drinks in a day. For women and for adults over 65 the threshold is four.

Unhealthy use is defined by the same countable logic rather than by impression. For men under 65 it means more than 14 drinks in a week or more than four in a single day. For women and adults from 65 it means more than seven in a week or more than three in a day. A standard drink is about 12 grams of ethanol, which is roughly a small glass of wine, a single measure of spirits or a third of a litre of beer.

One practical finding shapes how the question is asked: people report alcohol more accurately on a written or self-completed form than when a staff member asks them face to face. So the question comes in writing first and is discussed afterwards.

What about mood?

Routine screening for depression is recommended for all adults, and it is done with a validated two question tool rather than by asking how things are going.

The two questions are whether, over the past two weeks, you have often been bothered by feeling down, depressed or hopeless, and whether you have often been bothered by having little interest or pleasure in doing things. A yes to either counts as positive, and a positive answer leads to a longer questionnaire, the PHQ-9.

The table gives what each step actually detects, which explains why a positive screen is a question rather than a diagnosis.

What the depression screening tools detect
Step How accurate it is
Two question screen Sensitivity about 95 percent, specificity about 65 percent, so it rarely misses a case and frequently flags people who do not have one
PHQ-2 at a cutoff of 2 or more Sensitivity 91 percent and specificity 67 percent across 48 studies and 11,703 people
PHQ-9 at a cutoff of 10 or more Sensitivity and specificity both about 85 percent. At a 10 percent prevalence, a score under 10 makes major depression very unlikely, while a score of 10 or more still identifies the condition in fewer than half of the people who reach it

That last number is the reason a screen is never treated as a diagnosis. The US Preventive Services Task Force recommends screening all adults, and an individual participant data meta-analysis of the PHQ-9 published in the BMJ set out the accuracy figures above. A positive result is followed by a proper clinical assessment, including a direct question about suicidal thoughts, not by a prescription.

Screening only helps where something is in place to follow it up. That is why it is not done as a box tick.

Which blood tests are deliberately not ordered?

Four groups, each for a stated reason. Where you have the relevant risk factor, the test is ordered.

Tests left out unless there is an indication, and why
Test Why it is not routine When it is ordered
Thyroid function There is no evidence that finding and treating subclinical hypothyroidism in adults without symptoms improves outcomes Goitre, autoimmune disease, previous radioiodine or neck radiation, family history of thyroid disease, or a medication that affects thyroid function
Vitamin D Broad population screening is not recommended Very limited sun exposure, obesity, osteoporosis or malabsorption
Iron studies Not a test for every adult Premenopausal women, particularly with heavy periods or previous pregnancies, and anyone with a reason for blood loss or malabsorption
Resting ECG Not indicated in adults at low cardiovascular risk, even as a baseline Symptoms, or an established cardiac indication

The same applies to a group of blood markers that appear regularly in premium panels. Additional testing should answer a specific risk question rather than form an automatic premium panel. Lp(a) is different: the 2026 ACC/AHA guideline recommends measuring it at least once in adulthood. They have a place at intermediate risk, where a more definite estimate would actually change what happens next.

There is now direct evidence for what happens when low-value tests are added to a routine check. In a study of matched primary care patients in Ontario, an ECG in a low-risk adult after an annual health examination was followed within 90 days by an extra 1.92 cardiology visits, 5.49 echocardiograms and 4.46 cardiac stress tests per 100 patients. A chest X-ray in a low-risk adult was followed by an extra 1.96 CT scans of the chest or abdomen per 100.

Multi-cancer blood tests can produce false-positive and false-negative results. Performance differs by test and study; detection accuracy alone does not prove that routine screening reduces cancer deaths. What the trial data show.

What gets checked that packages usually skip?

  • Vaccination status against the adult schedule, which the US Centers for Disease Control and Prevention updates every year. This is one of the most commonly omitted items in an executive package.
  • A comprehensive eye examination from 40, for glaucoma, and earlier where there are significant risk factors.
  • Alcohol, formally, every year.
  • Family history on both sides of the family, with ages, which changes cancer screening more than any single test does.

None of these needs an expensive test. All of them need someone to actually ask or measure properly.

Executive health assessments in Dubai and the UAE

Is the UAE residence visa medical a health check?

No. The residence visa medical is a communicable disease screen. According to the official UAE government portal, every foreign national applying for a residence visa must test negative for communicable diseases such as HIV and tuberculosis. Certain worker categories are additionally tested for syphilis and hepatitis B, and female domestic workers for pregnancy.

It does not measure blood pressure to diagnostic standard, calculate cardiovascular risk, screen for depression or alcohol, check vaccinations or look at family history. Dubai Health lists blood testing and a chest X-ray in its medical-fitness examination. The current requirements depend on the visa and applicant category; a visa medical does not replace an individual preventive assessment.

This matters because it produces a widespread and reasonable misunderstanding. A resident who has passed a visa medical every two years has been tested for a small number of infections, not checked.

How common is high blood pressure in the UAE?

Around 31 percent of adults in the UAE have high blood pressure, and only about 29 percent of them are aware of it. Those figures come from a systematic review and meta-analysis of 15 studies covering 139,907 adults in the UAE, using a threshold of 140/90 mmHg.

The same analysis found the pooled prevalence higher in Dubai, at 37 percent, than in Abu Dhabi at 29 percent. Of those with the diagnosis, about 31 percent were on treatment and about 38 percent had their blood pressure controlled.

Read together with the awareness figure, that is the argument for measuring blood pressure properly rather than quickly. The most common serious finding in this age group is a number that nobody has confirmed.

Risk factors show up younger in this population than the age thresholds in most protocols assume. The regional picture.

Should vitamin D be tested in the UAE?

More often here than the general rule suggests, because the risk factor that triggers testing, very limited sun exposure, describes a large part of the working population in this climate.

In a study of 12,346 people in Abu Dhabi, 72 percent were vitamin D deficient at a level below 50 nmol/L and only 4.1 percent had a normal level. Levels were lowest in July and August, the months when people spend the least time outdoors.

So the honest position is neither of the two common ones. It is not a test for everyone by default, and it is not a test to refuse on the general rule either. Indoor work, air conditioned commuting and covered clothing are the specific reasons that make it reasonable here.

Your first consultation

Sit down with Dr. Felix, say what you want to know and leave with a clear plan. One physician, from the first question to the follow-up.

Who Is a Good Candidate?

  • You want the basics done properly rather than a long list of extras
  • You have been told you have high blood pressure on a single clinic reading
  • You have never had your vaccination status reviewed as an adult
  • Alcohol has crept up and you want it looked at without a lecture
  • You are 40 or over and have never had an eye examination for glaucoma
  • An active symptom, which needs a diagnostic appointment rather than a screening one
  • Chest discomfort or breathlessness on exertion
  • Thoughts of self harm, which need same day assessment

What Happens During the Consultation

  • 1

    History, including the awkward parts

Alcohol with a countable question, mood and anxiety with validated tools, sleep, activity and sitting time, and family history on both sides with ages attached.

  • 2

    Measurements taken to standard

Blood pressure measured properly and confirmed outside the clinic before any diagnosis. Weight, waist, and a focused examination.

  • 3

    Bloods that are actually indicated

Lipids, glucose or HbA1c, kidney and liver function. Thyroid, vitamin D and iron only where you have a specific risk factor, and you are told why they are or are not included.

  • 4

    Screening, vaccination and a plan

Age appropriate cancer screening arranged, vaccination status reviewed against the adult schedule, cardiovascular risk calculated, and a review interval set.

Program Structure & Follow-Up

After the assessment

You get the results with an interpretation, including what each borderline value means and whether it needs anything doing. A borderline result is named as borderline rather than reported as normal or abnormal.

What gets repeated and when

The table gives the interval for each item in this assessment. The intervals for screening tests are listed separately.

Repeat intervals for the assessment itself
Item Interval
Blood pressure Yearly from 40, every three to five years from 18 to 39 if normal and no risk factors
Alcohol screening Yearly
Mood screening At each preventive visit
Cardiovascular risk Every three to five years from 20, yearly at borderline risk or above
Eyes Periodic comprehensive examination from 40
General review Every three years under 50 without chronic conditions, yearly from 50

If blood pressure is borderline

The next step is measurement outside the clinic rather than a repeat visit or immediate treatment. Home or ambulatory readings settle far more of these than another appointment does, and they are the measurement the diagnosis is supposed to rest on anyway.

If a screen for mood or alcohol is positive

It leads to a proper assessment, not a label. A positive screen raises suspicion and nothing more: at a PHQ-9 cutoff of 10, fewer than half of the people who screen positive turn out to have major depression.

In men, low mood is reported less often than alcohol, risk taking or physical symptoms. How it presents in men.

Benefits, Limits & Safety

What this assessment is good at

Catching the things that are common, treatable and routinely missed: undiagnosed high blood pressure, unhealthy alcohol use, unrecognised mood disorder, missing vaccinations and undetected glaucoma.

None of these needs an expensive test. All of them need someone to ask or measure properly, and all of them are more common in this age group than the conditions that premium packages are built around.

What it will not do

  • It will not find every possible disease, and it does not try to.
  • It will not include tests without evidence for you, however commonly they appear in packages.
  • It will not reduce your mortality on its own.

The randomised evidence on general health checks is clear and it is not flattering. What it showed.

Why fewer tests is the safer option

Because the harms of screening are real and they arrive first: anxiety from false positives, complications of the investigations that follow, overdiagnosis of disease that would never have caused harm, and cost.

Patients tend to overestimate the benefits of tests and treatments and underestimate the harms, and so do clinicians, which is why both are stated in absolute terms here rather than as percentage reductions.

Thyroid, vitamin D and iron are tested when there is a reason, not by default, because tests without an indication mostly produce borderline results. Which tests are deliberately left out.

The point about follow through

Screening only helps where there is a system to act on the result. That is as true of a depression screen as of a mammogram, and it is the reason the plan includes who does what next rather than ending at the result.

The same clinician across visits changes outcomes more than a shorter interval does. The evidence for that.

The basics, done to standard

Blood pressure confirmed outside the clinic, alcohol screened annually with a countable question, mood and anxiety screened with validated tools, vaccinations reviewed.

Repeat intervals

Blood pressure yearly from 40, alcohol yearly, cardiovascular risk every three to five years, eyes periodically from 40.

Left out on purpose

Routine thyroid, vitamin D and iron testing without a risk factor, and any test whose result would not change your plan.

Cost & Program Investment

The consultation and each test are quoted before they are ordered. The panel is shorter than most executive packages because tests without an indication produce borderline results that then need chasing, and that chasing is neither free nor harmless. Vaccinations and the eye examination are arranged and quoted separately. Where a result needs confirming outside the clinic, home or ambulatory monitoring is arranged rather than another appointment being booked.

Frequently Asked Questions

Not on its own. Measurement outside the clinic is recommended to confirm high blood pressure before treatment is started. An office diagnosis needs an average of 130/80 mmHg or more from at least two readings per visit on at least two visits spaced over a week or more, and the threshold itself changes with the method: 125/75 for a 24 hour average, 130/80 for a daytime or home average.

No. According to the official UAE government portal, the residence visa medical screens for communicable diseases such as HIV and tuberculosis, with syphilis and hepatitis B added for certain worker categories. It does not calculate cardiovascular risk, confirm blood pressure, screen for alcohol or depression, or review vaccinations.

Because annual screening for unhealthy alcohol use is recommended for all adults, and because it is one of the highest yield questions in this age group. The question is a count over the past year rather than a general enquiry, and it is asked in writing first, because people report alcohol more accurately that way than face to face.

Because there is no evidence that finding and treating subclinical hypothyroidism in adults without symptoms improves outcomes. It is tested where you have a risk factor such as goitre, autoimmune disease, previous radioiodine or neck radiation, a family history of thyroid disease, or a medication that affects thyroid function.

More likely here than elsewhere. Broad population screening is not recommended, but very limited sun exposure is one of the specific reasons to test, and indoor work in this climate produces exactly that. In a study of 12,346 people in Abu Dhabi, 72 percent were deficient and levels were lowest in July and August.

Almost certainly some. Adult immunisation schedules are updated annually by the US Centers for Disease Control and Prevention and by national authorities, and vaccination status is one of the most commonly skipped items in an executive check.

A periodic comprehensive eye evaluation from age 40 for glaucoma, and earlier if you have significant risk factors. The evidence for screening is limited, but the examination is low risk and glaucoma causes no symptoms until damage has been done.

It leads to a proper assessment rather than a diagnosis. At a PHQ-9 cutoff of 10 or more, fewer than half of the people who screen positive turn out to have major depression, so the screen raises the question and the assessment answers it.

Because a resting ECG is not indicated in adults at low cardiovascular risk, even as a baseline, and because ordering one has measurable downstream effects. In a matched study of Ontario primary care patients, an ECG in a low-risk adult after an annual examination was followed within 90 days by an extra 5.49 echocardiograms and 4.46 cardiac stress tests per 100 patients.
