In short
Mounjaro is a weekly tirzepatide injection that acts on two gut hormones, GIP and GLP-1. In the SURMOUNT-1 trial, adults with obesity and without diabetes lost 20.9 percent of their body weight on average over 72 weeks on the 15 mg dose, compared with 3.1 percent on placebo. In Dubai, Mounjaro needs a doctor’s prescription and is usually a long-term treatment.
Treatment Overview
What is Mounjaro?
Mounjaro is tirzepatide, a once-weekly injection that activates both the GIP and the GLP-1 receptor. GLP-1 (glucagon-like peptide 1) and GIP (glucose-dependent insulinotropic polypeptide) are gut hormones that release insulin after meals, slow stomach emptying and reduce appetite in the brain. Clinical guidance describes dual receptor agonists such as tirzepatide as potentially more effective for weight loss than medicines acting on GLP-1 alone.
The European Medicines Agency licenses Mounjaro for type 2 diabetes and for weight management. In the United States, tirzepatide is also approved for moderate to severe obstructive sleep apnoea in adults with obesity.
Who can take Mounjaro for weight loss?
Adults can be considered for Mounjaro with a BMI of 30 or more, or 27 to 29.9 with at least one weight-related condition, when lifestyle changes have not produced about 5 percent weight loss. Clinical guidance prefers tirzepatide in most people who need obesity medication, because tirzepatide produced the largest average weight loss. For people with established heart disease and no diabetes, semaglutide is preferred instead.
Dr. Felix Lucian Happich decides after an examination, because diabetes, eye disease, other medicines and pregnancy plans change the answer.
How much weight do people lose on Mounjaro?
Adults with obesity and without diabetes in the SURMOUNT-1 trial lost 20.9 percent of their body weight on average over 72 weeks on 15 mg, compared with 3.1 percent on placebo. More than 2,500 adults took part, all with lifestyle support. The results for each dose are shown below.
| Group | Average weight change |
|---|---|
| Tirzepatide 5 mg | minus 15.0% |
| Tirzepatide 10 mg | minus 19.5% |
| Tirzepatide 15 mg | minus 20.9% |
| Placebo | minus 3.1% |
On 15 mg, 91 percent of participants lost at least 5 percent of their weight (35 percent on placebo) and 57 percent lost at least 20 percent. The results were sustained after three years. Clinical guidance judges the response after three months on the highest tolerated dose; less than 4 percent weight loss usually leads to a change of treatment.
How is the Mounjaro dose increased?
The Mounjaro dose starts at 2.5 mg once a week and rises in 2.5 mg steps, at least four weeks apart, up to 15 mg. The table lists the possible dose steps and the minimum time on each.
| Step | Weekly dose | Minimum time |
|---|---|---|
| Start | 2.5 mg | 4 weeks |
| Step 2 | 5 mg | 4 weeks |
| Step 3 | 7.5 mg | 4 weeks |
| Step 4 | 10 mg | 4 weeks |
| Step 5 | 12.5 mg | 4 weeks |
| Maximum | 15 mg | Ongoing |
Clinical guidance uses the lowest dose that achieves the desired weight loss, so many patients stay below 15 mg. People with diabetic retinopathy increase the dose more slowly and have their eyes monitored.
Should you choose Mounjaro or Wegovy?
Mounjaro and Wegovy differ in active ingredient, average weight loss and proven heart benefit. The table compares the two weekly injections.
| Mounjaro | Wegovy | |
|---|---|---|
| Active ingredient | Tirzepatide | Semaglutide |
| Hormones targeted | GIP and GLP-1 | GLP-1 |
| Average weight loss in the main obesity trial | 20.9% at 15 mg over 72 weeks | 14.9% at 2.4 mg over 68 weeks |
| Proven fewer cardiovascular events without diabetes | Not confirmed | Yes, in the SELECT trial |
More weight loss does not make Mounjaro the better choice for everyone. For adults with established cardiovascular disease and no diabetes, Wegovy is preferred because semaglutide has proven cardiovascular benefit in that group.
Mounjaro in Dubai and the UAE
How should Mounjaro pens be stored in Dubai heat?
Mounjaro pens belong in the refrigerator at 2 to 8 °C, according to the US prescribing information. If needed, a pen can be kept unrefrigerated for up to 21 days at temperatures not above 30 °C. Dubai summer heat, for example in a parked car, exceeds that limit, so an exposed pen can no longer be relied on.
Does Mounjaro affect the contraceptive pill?
Mounjaro can reduce the reliability of oral contraceptives in the first weeks and after each dose increase. The US prescribing information advises switching to a non-oral method, or adding a barrier method, for 4 weeks after starting and for 4 weeks after each dose increase.
Can Mounjaro be used during Ramadan?
Mounjaro can usually be continued during Ramadan, but should not be started in the weeks just before the fast. Clinical guidance for people with diabetes supports continuing GLP-1-based medicines during Ramadan, because nausea and vomiting are most common early in treatment. Studies come mainly from people with diabetes.
Fasting without fluids raises the risk of dehydration. People who also take insulin or sulfonylureas need a separate plan, because fasting raises the risk of low blood sugar.

Your first consultation
Sit down with Dr. Felix, say what you want to know and leave with a clear plan. One physician, from the first question to the follow-up.
Who Is a Good Candidate?
- Adults with a BMI of 30 or more, or 27 or more with a weight-related condition such as type 2 diabetes, high blood pressure or sleep apnoea.
- Adults with a limited result on a GLP-1 medicine such as semaglutide.
- Adults with type 2 diabetes who want blood sugar and weight treated together.
- Adults ready for a gradual dose build-up over several months and regular reviews.
- Personal or family history of medullary thyroid cancer, or multiple endocrine neoplasia type 2 (MEN 2): Mounjaro is not used.
- Pregnancy, plans to conceive soon, or breastfeeding: Mounjaro is not used.
- Severe inflammatory bowel disease or gastroparesis: GLP-1-based medicines are usually avoided.
- Previous pancreatitis: considered only if the cause has been resolved.
- Established heart disease without diabetes: semaglutide is usually preferred.
- Requests for compounded tirzepatide or a prescription without examination: not offered.
What Happens During the Consultation
-
1
Goals and history
Dr. Felix reviews weight history, previous medicines including any GLP-1 treatment, pregnancy plans and the contraceptive method, because Mounjaro can reduce the reliability of the pill.
-
2
Medical review
The review covers examination, weight, blood pressure, blood tests and all current medicines, including insulin or sulfonylureas that may need a lower dose.
-
3
Suitability decision
You receive a clear yes or no. When semaglutide or another option fits better, for example with heart disease and no diabetes, the reason is explained.
-
4
Personal plan
The plan sets the dose steps, nutrition and protein targets, strength training, what to do about nausea and the review dates for the first 12 weeks.
Program Structure & Follow-Up
How is Mounjaro treatment monitored?
Mounjaro treatment is monitored every 4 to 6 weeks while the dose rises, then about every 2 to 3 months, in line with clinical guidance. The reviews cover the following points:
- Tolerance: nausea, vomiting, diarrhoea, constipation, reflux, and symptoms of pancreatitis or gallbladder disease.
- Weight and health: weight, blood pressure and blood tests at agreed points.
- Diabetes: blood sugar checks during dose increases for people on insulin or sulfonylureas, and eye checks with diabetic retinopathy.
Between appointments, patients can contact the practice through the membership programmes.
Should the Mounjaro dose be reduced once weight is lost?
Reducing the Mounjaro dose after weight loss led to more weight regain in a 52-week trial. In that trial of 378 adults, weight stayed 21.9 percent below baseline on the maximum tolerated dose, 16.6 percent below on a reduced 5 mg dose and 9.9 percent below on placebo. Regain of at least half the lost weight occurred in 11 percent on the full dose and 25 percent on the reduced dose. Clinical guidance therefore keeps the established maintenance dose unless weight falls below the goal.
How is muscle protected during Mounjaro treatment?
Muscle is protected through enough protein, regular strength and aerobic training and body composition checks. How much of that is muscle, and what protects it, is covered on the GLP-1 page.
What happens after stopping Mounjaro?
After stopping Mounjaro, weight usually returns and the metabolic benefits reverse, according to clinical guidance. A meta-analysis of 37 studies with more than 9,300 participants found an average regain of 0.4 kg per month after stopping obesity medication, with a projected return to baseline within two years. Options for keeping weight off are described on the weight maintenance page.
Benefits, Limits & Safety
What are the side effects of Mounjaro?
The most common side effects of Mounjaro are nausea, vomiting, diarrhoea and constipation, usually mild to moderate and worst after a dose increase. Across GLP-1-based medicines, 25 to 40 percent of patients experience nausea and about 10 to 20 percent vomiting, diarrhoea or constipation.
Serious but less common problems include gallbladder disease, pancreatitis and bowel obstruction; a direct cause has not been proven for all of them. People taking insulin or sulfonylureas can develop low blood sugar as weight falls.
Tirzepatide caused thyroid C-cell tumours in rodents. Human studies of GLP-1-based medicines have not confirmed an increased thyroid cancer risk, and routine calcitonin tests or thyroid ultrasound are not recommended.
How can nausea on Mounjaro be reduced?
Nausea on Mounjaro often improves with smaller meals, slower eating and a delayed dose step. Clinical guidance lists these measures:
- Eat smaller, more frequent meals.
- Stop eating at the first sign of fullness.
- Limit fatty, greasy and spicy food.
- Delay the next dose increase by four weeks if nausea persists.
When should you seek urgent medical help on Mounjaro?
Seek urgent medical help on Mounjaro for severe abdominal pain, persistent vomiting, signs of gallbladder problems or sudden vision changes:
- Severe abdominal pain that spreads to the back.
- Persistent vomiting that prevents keeping fluids down.
- Pain under the right ribs, fever, or yellow skin or eyes.
- Sudden vision changes in people with diabetes.
In the UAE, call 998 for an ambulance in an emergency.
What should other doctors know about Mounjaro?
Other doctors need to know that Mounjaro slows stomach emptying and can lower blood sugar together with diabetes medicines.
- Surgery or sedation: Mounjaro may need to be paused before a planned operation because of the risk of aspiration during anaesthesia.
- Diabetes treatment: insulin and sulfonylureas are often reduced during the first four weeks.
Why are compounded tirzepatide and microdosing not offered?
Compounded tirzepatide and microdosing are not offered because neither has been tested for safety and effectiveness. Compounded copies do not go through regulatory review, and safety problems have been reported. Deliberately using smaller doses than prescribed has not been studied and carries a risk of dosing errors.
Only buy these medicines from licensed pharmacies. What the Emirates Drug Establishment did about unapproved peptide sellers is described on the prescription weight loss page.
What the evidence shows
20.9 percent average weight loss over 72 weeks on 15 mg in SURMOUNT-1 (placebo 3.1 percent), sustained at three years. Individual results vary.
How you are followed
Reviews every 4 to 6 weeks during dose steps, then about every 2 to 3 months, covering tolerance, weight, blood pressure, blood tests and diabetes medicines.
When Mounjaro is not used
Pregnancy, breastfeeding, personal or family history of medullary thyroid cancer or MEN 2. Usually avoided with severe inflammatory bowel disease or gastroparesis; previous pancreatitis needs individual review.
Cost & Program Investment
What Mounjaro treatment costs depends on the dose and the length of treatment. Dr. Felix explains the figure before any prescription.
Frequently Asked Questions
Mounjaro produced more average weight loss than Wegovy in the main obesity trials (20.9 versus 14.9 percent). Wegovy is preferred for adults with established heart disease and no diabetes, because semaglutide has proven cardiovascular benefit in that group.
Tirzepatide activates the GIP and GLP-1 receptors, while semaglutide activates only the GLP-1 receptor. The dual action is linked to larger average weight loss in clinical trials.
Adults can be considered for Mounjaro with a BMI of 30 or more, or 27 or more with a weight-related condition, when lifestyle changes have not worked. A doctor in Dubai decides after an examination.
Mounjaro usually reduces appetite within the first weeks, while weight loss builds as the dose rises. Clinical guidance judges the response after three months on the highest tolerated dose.
A missed Mounjaro dose is taken as soon as possible within 4 days, according to the US prescribing information. After more than 4 days, the missed dose is skipped and the next dose is taken on the usual day.
Mounjaro can make oral contraceptives less reliable. The US prescribing information advises a non-oral method or an added barrier method for 4 weeks after starting and after each dose increase.
Mounjaro pens are stored in the refrigerator at 2 to 8 °C. The US prescribing information allows up to 21 days unrefrigerated at no more than 30 °C, which Dubai summer heat can exceed.
Mounjaro is not used during pregnancy or breastfeeding according to clinical guidance for GLP-1-based medicines. Pregnancy plans and contraception are discussed before starting.
Lowering the Mounjaro dose after weight loss led to more regain in a 52-week trial. Clinical guidance keeps the established maintenance dose unless weight falls below the goal.
After stopping Mounjaro, appetite returns and most people regain weight. Obesity medication is therefore usually long-term treatment when it works and is tolerated.
The cost of Mounjaro in Dubai depends on the dose and the length of treatment. Dr. Felix explains the full cost in the consultation before any prescription.
